Did the second set of cab but had to stop due to it stopped working, lymph nodes increasing in size and liver lesions increased in size. The oncologist is scheduling a liver biopsy to see what type of prostate cancer it is, no idea about the procedure, had anyone had this done ? Will the cancer sensitized to earlier treatments that failed in the past ? He is checking into trials fir plevitco , live in Central New York but the only one he knows of is in New york city , 6 hour drive and I am single. Is there anything that would prolong my life at this point, he says too much going on for radiation, or is this the end ? Been battling this disease for 5 years, frustrating and discouraged. The scrotum is swelled so tough walking and cannot do alot of walking, they are working on a sling, any body experienced this ?

Is driving to New Haven any better? Dan Petrylak at Yale is excellent and may have a clinical trial there.
I will check it out, any thoughts on the other questions in the post , can the cancer be re sensitized to other chemo or zegtia ? You have been a wealth of information , do not know what the site would do without you.
There are many BiTE trials now that you can ask about. Maybe there will be some clues from the liver biopsy.
Thats a question I have in regards to your current status. Docetaxel even if it is stopped after a few infusions due to not working still resensitizes so that Xtandi can be tried again. I don't know if that's the case with the Jevtana/Carbo you had.
Thank you, I think he might do that after the liver biopsy to find out what type of prostate cancer it is, Zigtea only lasted a year and half and xtandi was a bust last July of 2022.
If the chemo you had resensitized the PC cells (AR's) Xtandi might work for a few months as a bridge until you can start Pluvitco and get a handle on what and how to treat your liver mets ( you mentioned your Dr. looking into a Pluvicto trial, unless it is a trial combining Pluvitco with another treatment Pluvicto is FDA approved and insurance covers it no need to be in a trial to get Pluvitco ).

The drug is in short supply probably that is why he is looking for trials that can get it easier. Doing research but after you get the plivicto then how do you maintain without the cancer coming back, what drugs do they use ? I cannot find anything about what they use after the treatments to maintain the cancer from growing again.

I am 3 years into this started with chemo, then Zytiga, then chemo again, and started Xtandi a couple months ago. Pluvitco will be next for me when Xtandi fails. I asked my MO if Pluvicto could be given more than once-(yes). Asked if some of the treatments I have not had could be given after pluvitco (Ra-223, Provenge, Jevtana--yes).
Any treatment can be a failure right. I try to thread that needle of being optimistic about the next treatment but not devastated if it fails. Easier said than done maybe. But hey were all different and this is not easy. Starting out on Docetaxel, then about 10 months later needing Zytiga. When Zytiga started to fail even after switching to dexamethasone I had that lump in my throat, oh shit feeling, that I've run thru 2 good treatments and the next treatments will probably be fast and fewer from here on out. Sure enough a rechallenge of Docetaxel didnt work and so now on Xtandi. I know your head isn't in a good place but have some faith. Hang in there. I think you'll get some more time from the next treatments.
Xtandi might be a bridge before you start a trial also. Sometimes getting into a trial can take a bit of time with labs, scans, questionnaires' etc. I don't know. Getting with the MO's recommended here might put you in the best place for decision making (trials, treatment choice etc.).

My husband had a liver biopsy in 2018 and it's a similar process as the prostate biopsy. He felt no pain after the procedure. I'll let the general wisdom of the group answer treatment questions, but I'll urge you to check Amtrak schedules for traveling into the city if you need to do so. There may be a station close or a short drive from where you live in central NY. The Empire Service station points are: Buffalo - Rochester - Syracuse - Albany - New York. If need need assistance on the train, they'll provide it. Other train lines may get you to other NE locations as TA recommends checking in with Yale. I would request an appointment with Dr Beltran at Dana Farber in Boston. She works in this type of PC not responding to conventional therapy and she could guide you to clinical trials,
