Chemo for Pelvic Lymph Nodes

Hi all,

When I was diagnosed with metastatic prostate cancer 3.5 years ago, I had small mets scattered on my spine and mets in prostate and pelvic lymph nodes. The ADT has eliminated the spine mets, but the lymph nodes continue to grow. Will Chemo kill the lymph node mets or is it more effective for bone mets?

According to this, chemo was not significantly effective when there were LN metastases:

Thank you T_A for reference to the study. I am now castrate resistant. Does this make a difference since the study involves castrate sensitive patients? I’m trying to decide if it makes sense to do the docetaxel chemo first so that I am eligible for Pluvicto. If the chemo is helping, then I will continue with it.

I think that rapid growth is usually a signal that chemo will benefit.

I'm curious about your comment that "The ADT has eliminated the spine mets ...". I'm going into my 6th month of ADT (Eligard, abiraterone/prednisone), and had the strong impression that while it might shrink my tumors (prostate and pelvic lymph nodes) ahead of EBRT, it wouldn't actually make them go away.

What I meant to say is that the spinal Mets do not show up on bone scans after ADT. I’m sure they are still there, however, have shrunk to a size that are not picked up on scans..

I had SBRT on an enlarged lymph node on the iliac chain. Actually I had it done twice, one year apart as the first time didn't seem to do the trick. Treatment worked well for me.

Every individual case is different. You don't know for sure which treatment(s) will work until you try them. Each treatment decision weighs potential benefits with potential side effects.
