Once your cancer has metastasized, surgery (or radiation) is rarely performed. You need systemic treatment. However I believe there are some trials that start with sytemic treatment and then surgery. MD Anderson maybe.

In UK, if you have under a certain number of mets they will radiate the prostate. In Finland, my husband had SBRT to the mets also. As you are in Malaysia, you could check out LU177 in Thailand which is non SOC for hormone sensitive men but is something my husband and several people on this site have tried.

I’m in a similar situation with lymph nodes and one bone met. I’m in the US and outside of a clinical trial they said no surgery. This was hard for me to hear as I’m younger and in otherwise excellent health.

I am on ADT and darolutamide (as well as having completed chemo) and am now getting EBRT to prostate/SVs, pelvic lymph nodes and my one bone met.

Considering my GS of 9, my RO said that I’d need radiation anyway even if they would do surgery. Because of this, many guys would opt to not do surgery and deal with more side effects. This was a much better explanation than what I got from the urologist which was basically “no surgery for you!”

I’m much more comfortable now in just having radiation and all the other systemic therapies I am receiving.

My prostate exploded with pc tumors . Surgery was never an option . I’m happy that I skipped it .

I know a guy that had Da Vinci that nicked his colon and drove him to get mesh and hospital trips nearly killing him . I’m sure he’s an exception to the rule . There is risks in everything . He got some money but not enough for what he suffered . Most MO’s will not operate with bone metzs …There is no point in it at that stage . I was non op too . imho
