HELP, Anyone here with advanced metastatic prostate cancer on a inhibitor, biologics for RA or Psoriasis or for another reason?

Please help, trying to find out more information

The reason why I’m asking

My husband has sever plaque psoriasis and

Advanced metastatic prostate cancer

AMPC Gleason 8 ductal w/ crib form

4 cores /15 diagnosed 7/2022

He was put on otelza his PSA rose in 5 days 

So he stopped it for 2 days now back on 

He took Tremfya for years and Stelera before that

 Wondering if he should be on a IL-17 biologic or IL-23 inhibitor instead 

MO did not want him on any suppressant but it’s not possible 

Head to Toe, his skin is the worst case ever seen in many dermatologist offices

I am on Cosentyx. Approved by my oncologist. I am stage 4, currently NED, five years after diagnosis.

Thank you, We are trying to figure out as it seems most dermatologists don’t know if a pde4 or IL-17 or a IL-23 inhibitor is best with pc as he also has mutations, so we are researching what is best between these

It’s interesting about PSA rise on PDE4 inhibitor Otezla. Clearly stay away from that. An IL-17 inhibitor sounds like it is worth a try as long as you follow PSA response. I have been wondering if these drugs could actually be beneficial in PC by strongly lowering inflammation? (I do not have psoriasis, thankfully) Paul

Actually some studies are being done with IL-17 but I’m leaning towards a IL-23 inhibitor

That is indeed the question: which, if any, inflammatory and immune modulators might be of benefit without undue risk of excessive (severe) immune repression? As inflammation is also a key driver of cancer progression. (I do not have psoriasis or other autoimmune disease.)

Yes, I have both. I'm taking Skyrizi for the psoriasis; it works extremely well. What is your question?

Some thought on Psoriasis

I have been helping a young woman with this. she said she scratched so much she would bleed.

I suggested she is probably allergic to something she is eating, and to go to an allergist that would test her specifically for this,

The main culprits for food allergy are wheat, eggs, nuts and chocolate,

she came back and said yes, she is allergic to many foods.

for help with your husband's skin, applying some aloe vera on it several times a day might help until you actually know what is causing it,

those drugs are just to cover the outbreaks but finding the reason for the outbreaks seems to be something that might help....

You're welcome. \If the allergist does not find any food allergies, there might be other things he is allergic to. this is a tough situation.🌹
