PSA Test Frequency After Extended ADT

I'm curious what PSA testing frequency others have had following long ADT treatments.

A little background on me:

- Diagnosed summer 2019 w/G9 at age 56

- RP Aug 2019 confirmed G9, ECE, SVI+, LN+ so stage T3cN1

- Decipher score was 0.71

- Had 35 sessions of IMRT in summer 2020 along with 24 mths of ADT (started in spring 2020).

The ADT began wearing off in April 2022. My most recent bloodwork was last month (Oct 2022) where my PSA was still undetectable (<0.008) and T=118. The PA told me that I would have another follow-up in 3 months (Jan 2023) but then move to a 6 month test schedule.

Maybe I'm overly worried but I would rather continue testing every 3 months for at least another year before considering a reduced testing frequency.

What testing frequency have others experienced following their primary treatments?

On a clinical trial, I did every 3 months for one year, then every 6 months for one year, and once annually since then. I think I increased it to every 3 months when I had bounces. PSA tests always cause anxiety and normal fluctuations gain oversize importance in one's mind.

So far, it looks great for you.

3 months would be awesome...a break from the monthly "anxiety".Jim's been 20 months on Abiraterone and 22 months on Lupron. Been <.04 since Oct 2021 (<0.02 on two ultra sensitive) but the MO still wants monthly. T <10

I'm not sure how comfortable 6 months would be, but TA is the one that carries the most weight in my mind.

Alternative scheme: Monthly until your T gets over 350. After this and provided there is no evidence of an upward trend, extend the testing period by half to one month each time the latest PSA value is within +/-20% of the preceding one. Equally shorten the testing period in the opposite case.

I would test every 3 months for at least 1 year since the Gleason is 9 and the T is increasing but below normal values at this time.

Blood tests every three months for 2+ years. I'm in a clinical trial maybe that's why. Scan frequency has gone from 3 months to 6 and now to 12.
