My psa again came back at 0.1 for the second time is this good or bad... Started at 79 in in January 2021...on Lupron and xtandi should i be doing anything else...20 rounds of radiation to prostate in September 2021...

I'm coming up on the 3 year mark for Lupron / Xtandi (1/2 dose) combo... PSA started at 2400+ was down as low as .06 now hovering between .13 and .2 .

My dad, 78, after 7 months on Abiraterone and prednisone just got a second PSA of 0.1 He now says he thinks he's going to live to be 100. However he is suffering a lot of muscle wasting and, try as I might, he refuses to do strength-building exercises. He is still farming as he's done for 60 years.
He was diagnosed in 2018. Had RP, chemo, ADT. But in December 2021 his PSA started climbing again and he was diagnosed with Mets to bones and lymph nodes, and was put on Abiraterone.
Thinking that he will live to be a 100 is the best state of mind to getting there. The demon of despair that feeds on the fear of impending death destroys the pleasure of living in the moment. Coming from a long line of farmers, I wager a good deal of exercise is already built into his day.

This is interesting. The above link is from another member here. It seems the prevailing expert view backed by evidence consider PSA under 0.2 as 'cured' for 98% or LT (10 years) good result. The reason being that with the prostate in place, there is bound to be some PSA. Others , notably Dr. Scholtz maintains that the low point has to be under 0.1 and above 0.05 to be safe. The reason is that you cannot be certain there is no cancer in the body. Interestingly, he also maintained that level should be achieved in less than 7 months. Another issue with the prevailing opinion.
Who is right? Time will tell. May be.
The information from Dr. Scholtz is very interesting. I had a prostatectomy in November 2018 and my PSA has risen slowly to from 0.0 to 0.1 over four years. I have a follow up blood test in February 2023 and I am praying and hoping that there are no further increases. The doctor’s point about achieving what he considers to be a safe zone within seven months validates my thoughts that it’s important to do what you can to keep your PSA levels down through diet and exercise to avoid further treatments.

Not fully understanding: the low point has to be under 0.1 and above 0.05 to be safe...my last 3 psa tests were <0.01...do I need to be concerned; why above 0.05 to be safe; I thought a low number is what we would want, could you explain?

The issue is in the age of informed patient choice, and what is the medical team advice. Some doctors will say that PSA 1.0 is disease free. Further a normal person with prostate intact can have PSA of 2.0. But he may have cancer cells. That is partly because there is no technology to detect all prostate cancer below that level. So at what point do you say the man is cancer free?
Also it seems there is some confusion with undetectable PSA. Some seem to imply that unless your PSA is undetectable, be it 0.03, or 0.0002 you are not disease free. That cannot be right for the reason give above.
