Just started my first round of Pluvicto a few weeks ago. When can I expect to see a PSA drop? After two weeks I saw a rise by 300.

When Cancer cells get killed a large numbers by any treatment, Initially there is sudden, big rise in PSA which later falls precipitously . I hope this turns out to be true in your case.

2 weeks is too early - lots of dead cancer cells.

Hi, TA,

My husband will be getting Act225 treatment in Munich Germany in early September.

I am wondering what measures should we do to minimize the saliva gland damage during his treatment.

Best,

Macdunno

Roughly one half (40-60%) of patients will experience a drop in PSA of 50% or more. This is a favorable indicator for longer overall survival and longer PFS. But some don’t see the drop until after the second treatment. Persist!

Husband saw a drop from 25 to 3.85 on first treatment. Dropped to 0.8 the next 3 treatments . After 6 isotopes the psa has been doubling every 3 weeks. Back up to 16 after last isotope mid june. My theory is that once off the pluvicto the psa rises again quickly. But he had a very good year with very little side effects from pluvicto. Much less than chemo. And of course everyone is different.

Average time to progression after a good initial response is 13 to 16 months. Of course none of us are the average. Perhaps he should have an FDG PET scan to see if non-PSMA expressing cells have taken over. Would need a different approach now.
