Is there any reason my MO will not Rx estradiol patches for hot flashes and bone density?

6 months into Orgovyx , Zytiga and prednisone and I finally watched Snuffy Meyers’ YouTube recommending them .

Nope , but he hasn’t suggested it and I’m requesting an Rx tomorrow. Just want to be prepared if he won’t write it .I’ve tried Oxybutynin , Gabapentin and 2 types of Effexor . They all seem to help with hot flashes but I can’t tolerate them for different reasons .

MSK MO’s nurse called in Rx today without argument or discussion . I promised to get blood tests to monitor .

I read a recent comment on here saying that since it’s not SOC uros won’t prescribe.sending request for Rx today . My naturopath would probably write it for me or I can buy OTC transdermal cream . Thanks for checking in. I’ll let you know what happens.

They will not prescribe it here either in OZ for some reason, they prefer first line treatments like ADT that are money spinners for them. I am using the cream & have a blood test soon to see how, or if, it keeps the PSA down.

Hope it has worked . I’m on ADT to keep PSA down . I want to try the cream or patches for ADT side effects .

I was on Firmagon ADT & it reduced the PSA to 0.3 last blood test 3 months ago so I am trying the cream now to keep it down. Next blood draw is early June so will let you know if it works.

I was lucky enough to order patches for women having a period. I will read the fine print closely.

He may try and tell you that it will increase cardiac risk which isn’t true. Ask for his proof of that. While that may be true of estradiol in pill form which is metabolized through the liver it is not true of patches that are metabolized through the skin.

Yes, both my MO and urologist refused to prescribe estradiol patches due to risk of blood clots they said. Do you have a study I could show them to try to change their mind? 11 months of Xtandi led to massive muscle loss and fatigue and would like to feel better.

Estradiol won’t reduce fatigue or muscle loss, I exercise daily to maintain muscle tone and it helps with fatigue. Some oncologists prescribe Ritalin for fatigue and cognitive issues, you can ask about it. Be sure you are taking Xtandi in the evening not it the morning. Lastly Xtandi can be effective at lower doses than 160mg, ask your MO. I’ve been on an 80 mg dose for about 3 years now per Dr. Sartor.

Why would estradiol not help with fatigue? I thought the lack of hormone is what causes the fatigue. It’s hard to exercise with such profound fatigue.

It’s the lack of testosterone that causes fatigue, estradiol obviously is not testosterone. In men some testosterone gets converted to estrogen/estradiol, so if you don’t have any testosterone to convert you get hot flashes like a menopausal woman who lacks estrogen. Snuffy Myers has several YouTube videos on estradiol patches I suggest you check them out.

There is no reason for your doc to keep you from getting patches. I talked my onc into doing it. My results were so profound, he now has the vast majority of his metastatic prostate patients on E2 patches. I do 100 mcg 2X a week. I’m on Prednisone and Zytiga. Had an orchiectomy from the start. All good. Nice looking knockers, too.

Profound results. I imagine with reduction of hot flashes and bone health.

Did you notice any mental changes? A shift away from brain fog and depression? Any changes in energy level?

NO hot flashes, flushes, etc. None. Went away. Nails, bones and hair all better. No longer depressed or having dark days. Only time I have brain fog is when I f-off and stay up reading or playing guitar. I’m able to exercise again as well. BIG diff, dude. I’m now a eunuch with benefits. Sex scenes in movies no longer trigger me.

I am a lifelong wanna be. Haven't got far at all so live vicariously thru those who do play guitar lol.

Speaking of can patches help clear up the brain fog lol.You cant believe how dense I feel missing that as a bass. Duh.

To make matters worse I am having Dejavu. I think I already asked you and had the same duh moment when you answered.

If I ask you again give me some hell lol.

Your bass and punk rock I am sure I would get into.

Rick Beato did a you tube video trying to scientifically identify the difference between several string thicknesses. Pretty cool.

Thanks for your input . I hope it will help me . If the MO won’t Rx patches I’ll get the cream. My libido is non - existent and my equipment has shriveled up . This ADT is not for wimps .

We requested them for DC but his oncologist refused saying there is not “proof” that the patches work. I asked he if we can just TRY them & if they don’t work then they don’t work, he just replied is not SOC. I’m still working on it though. There is NO REASON that, with everything else he’s going through, he needs to have these stupid hot flashes!! Keep us posted!

I talked my GP and Urologist into prescribing them and they worked great. However, when my Hematologist, and then my MO (Dr. Rettig) heard about it, they said stop immediately. I have a blood clotting disorder (Factory V Liden) and they said it can cause blood clots. I stopped and then tried Venlafaxine, which I am getting off of now due to some other issues. It definitely helped. Most of the other drugs to stop hot flashes they said are hormonal and they have some clotting issues as well. Not sure what is next.

Sorry to hear about the clotting issue making things more challenging. My MO Rx’d the patch so I’m going to give it a try .

I had done success with Venlaxafine and Gabapentin but the former makes me feel anxious and the latter makes me feel dizzy . I haven t tried Accupuncture or Mace yet . Waiting on reply from my MO about estradiol .

I took PATCH and other studies to my urologist to back up my request 2.5 years ago. He said it was "off the range", but I helped him justify low dose for side effects of a year on lupron. Still on lupron, but life improved a lot adding E2.

My MO Rx’d the patches today . Then I talked to a guy who said they didn’t help him during 8 months on ADT . Worst thing is he also said he stopped ADT six months ago and is still having hot flashes . Yikes !
