I had my first dose on feb 23rd and the first few days after treatment with no real issues, until, about day 4. I experienced nausea , vomiting, and lower back pain as well as rib and thigh pains accompanied by neck and cranium discomfort. My stomach is cramping some of these side effects are normal although some are unexpected. For those of you that have knowledge are have gone through Lu 177, is this normal, I cant seem to get a clear answer from the onc nurse. My labs were good except alk phos levels In the 800s. It was suggested that this may be a positive sign that the treatment is working perhaps wishful thinking but ok, it sounds good to me, hope is a beautiful thing.

You usually get a drug against nausea and vomiting at the time you get the Lu177 injection. I think the pain is caused by Lu177 attacking the cancer cells. I had no bone mets so I cannot report from experience.
Thanks for keeping us up to date on your situation. Have you been losing weight and having these side effects the whole time? Your clothes look baggy. I hope the study people are on top of things. You can probably get more help here based on personal experiences.
Sorry to hear your having such a strong reaction. I didn't have those effects during my three sessions. I felt pain in the areas that had shown the highest uptake in my PSMA PET scans, but I faded within a few days. I didn't have any nausea, but that might be related to the total cancer burden. It seems to me that the inflammatory chemicals released in cell death could cause the nausea.
My PSA and ALP continued to rise throughout all three treatments. It sounds like your blood tests were shortly after treatment c, so high ALP may be related to treatment effect. I think a blood test three weeks after treatment will give time for cancer cell detritus to flush from your system, and give you a better reading in effectiveness.
The half life of 177Lu is 7 days. My understanding of the physics is that each atom decays once, and releases a beta particle. So half of the Lu isotopes will decay once over the seven days. The remaining ones that are still bound within a cancer cell, will decay later. If beta particle released in the decay causes a double strand DNA break wiithin the cell, or a nearby cell, the affected cell will die when it tries to divide.
I think all this means that the effect of the treatment lasts weeks,,rather than the days of chemotherapy drugs with 12 hour half lives.

I had nothing after infusion 1 or 2 but vomited once on the journey home from infusion 3. I had had a flight and about 1 hour into the taxi journey home I had to stop the car and vomit. Straight to bed and was fine in the morning.
I had first chemo yesterday and flew home and no side effects.. yet probably!

Yes everybody is different & seem to have different adverse side effects with LU-177.My first infusion nothing happened but the PSA went from 18 to 3 but then on the second infusion I had blood coming from an old stomach ulcer & pain in right knee but PSA went to 1.6. I never had the third & was told to have chemo instead.
Did you have mets. What are currently doing?
May not apply, but when I had chemo some years ago that a saline infusion the day after treatment ended my nausea. Good luck with your treatment plan. I may be headed that way soon.

Hi costarica1961,My husband had nausea after each Lu177-injection. Only one drug helped him very good. In french it's called: chlorhydrate de metoclopramide anhydre. 1 pill of 10mg 30 minutes before each meal. It exists also in form of drops.

Phase III trial at Dana Farber, have completed 2 of 6 scheduled infusions. No ill side effects to date. Still getting Lupron every 3 months. PSA dropped from 8.1 to 1.8 six weeks after first infusion, then two weeks after 2nd infusion PSA 1.4. Bone and CAT scan show shrinkage in soft tissue, as well as bone mets. Steady as she goes, one day at a time.
Keep things positive. It Sounds like another kind of hell. Anything to kill cancer . Stay up! 🙏🍀
