Can anyone advise generally speaking is docetaxal with ADT the best way forward at the outset

Oncologist is saying that the addition of Abi + peridsone is not needed as low volume

We don't have the low volume data yet from PEACE1. That doesn't mean the combo is ineffective for low volume, it just means it's too early to tell. It also may be true that docetaxel+darolutamide is useful, but we haven't seen any of that data yet, just a press release.

Nobody knows, data is lacking. Get a second opinion and decide from that point.

Nick James stated that low volume patients that take docetaxal 80% of them will survive 5 years + causing concern over the longevity of men on ADT and the issues that can cause

They told me the same thing 7.5 years ago. I Started with chemo and have slowly transitioned to Zytiga. Hell you need to think in terms of at least 10 years.

My oncologist recommended ADT along with docetaxal or abi. I managed to get an appointment to see a prostate oncologist at Memorial Sloan Kettering in NYC. He recommended that I do all three based on the results from the recent trial. However, their decision was based on the fact that I’m 45 with no comorbidities. I’m in my 4th dose of docetaxal and my psa and ALP are down considerably. Your mileage may vary.

My MO Recommended all 3 also. So many recent phase 3 studies have proven these drugs given in combination and early lead to longer survival.

Not sure my situation is the same but after surgery and BCR failed, I visited Mayo in Jan 17, did the triple therapy based on a C11 Choline scan which showed four PLNs but no bone or organ involvement, so, "low volume." It's been about 3-1/2 years since I finished and the Lupron cleared my system, I see my urologist every 2-4 months for labs and consults but so far, no treatment decision required.
