Had surgery but PSA is climbing. PSMA scan confirmed activity in lymph nodes. Would like a second opinion. Suggestions?

Your alternatives are (1) to go for a cure with salvage whole pelvic radiation + ADT + hormone therapy or (2) to maintain the cancer with ADT plus advanced hormone therapy. For #1, you have to talk to a radiation oncologist. For #2, you have to talk to a medical oncologist. I suggest you talk to both and then decide. Meanwhile, start on ADT so that your cancer doesn't spread.

Consider this clinical trial:

Where are the lymph nodes located? Are they in the pelvis ?

Hi Tango,

Have a Dr appt on Weds to discuss PSMA scan. Preliminary discussion with Dr indicated cells in a couple of different lymph node locations. Will determine next step on Weds. Appreciate your info. Will post following Dr visit.

I live in Atlanta area but have had to travel to see a prostate specialist. I currently see Dr. Sartor in NOLA who coordinates with my general MO in ATL. I currently see Sartor every 6 months, he is excellent, one of the best in the field. And having a reason to enjoy some NOLA food every six months is a nice fringe benefit, lol.

Hi Ed,

Live in Senoia and have a "group" appointment with my MO and Radiologist tomorrow (Tues) for an initial discussion. May solicit a second opinion. Been doing a lot of research. Had Radical Prost 10 yrs ago, PSA level and then spiked to 8.0, did PSMA scan and found the little bastards in lymph nodes in a couple of places. Will know more later, will post. Thanks for the lead.

You may want to go see Dr. Busch at the Busch Center (Alpharetta). He has the latest 3T MRI available. He can do a multi-parameter MRI of the pelvic area or a whole-body MRI. He has decades of experience in dealing with prostate cancer.
