Hi Good People

I wonder some patients remove their prostate gland if its large enough when the Oncology can't treatted with radiation treatment . My urology he told me that.

I have never heard of that. You may want to get a second opinion on that.

Low dose rate brachytherapy can't be done on large prostates. All other types of radiation can be given regardless of prostate size. A urologist wouldn't know - it's not his field.

A urologist will usually recommend surgery instead of radiation. I think it is always a good idea to treat the prostate, either with surgery or radiation.

My urologist gave me the choice of surgery or radiation & I chose radiation but if I had that choice again I would have surgery. Now the cancer is in the blood stream & has travelled to lymph nodes& beyond.

Surgery can also leave some cancer. Whether you have surgery or radiation there is always the possibility of the cancer returning. I had brachytherapy for Gleason 3 + 3. Now, I am battling metastatic prostate cancer.

Yes, both surgery and radiation have high recurrence rates, so don't beat yourself up about your choice of treatment !!!

I had a RP, PSA went to <.02 after surgery . Less than a year later PSA was .60 with a BCR. PSMA scan showed lymph node involvement. Recurrence is a possibility either way.

I know somebody that had it removed as soon as he was diagnosed with PC & today he is healthy without PC & even sired two children. Maybe he was just lucky?They take the sperm with injection I believe & proceed with Artificial Insemination.

I believe that if you have radiation you can't have surgery later. Why reduce your possibilities? Get the surgery. Your prostate isn't doing you any good. Suffer the after effects of surgery. You still can have radiation if you need it.Mike

There is little/no evidence that modern day radiation +/- ADT (depending on risk grade) is less effective long-term than surgery......both primary treatments have salvage options.

I also had a large prostate. Radiation choice did not look good as my peeing was a slow go and after radiation it can get worse. Also for later in life a very low flow could mean a bag. My prostate is gone and boy have I got good flow now! I also have the choice of radiation if things go south.

During My first DRE the uro said” you’ve got a gigantic prostrate “ I was beyond Rp .. t-4 they told me “ the horse is out of the barn” I did Lupron with Tak-700 and imrt . Imrt can work with a gigantic prostrate . It did for me. Good luck !
