Has anyone experienced excruciating pain after a few weeks of starting Xofigo (Radium 223)? Started 08/04/2021. Hospitalized since Sat-8/21

Oncologist states they don’t see it is cancer related from CT Mylogram. They see degenerative disk disease which we know he has a past back injury from 1995. She states it could be from the Xofigo attacking the cancer cells. Thoughts?

He should be taking a bone-preserving agent (Zometa or Xgeva) if he is taking Xofigo.

There is often temporary painful bone inflammation when using radiation on bone metastases. Some steroids and NSAIDs can provide relief for the required 2 week period.

He is on Xgeva and Trailstar which replaced Lupron. They gave him a steroid pack in the hospital and the next day he was better. Be he is also getting Dilauted by IV. They have tried to wen him off so we can go home with a 100 MG Fetanyl patch and oxycodone 20 three times a day. Pain was severe today without the dilauted. Mainly lower back and leg pain. Increasing Fetanyl patch to 125 mg and we wait

One of the men in my clinical trial had necrosis of the jaw. He tried a number of different pain management ideas. He told me the thing that worked is gabapentin. I know nothing about this medication other then a fellow warrior though it was a miracle for him.

I have had more success with OxyContin extended release than I did with oxcodon 10 mg. For back pain from. RT relieved the pain and I cut out Dexamethasone which is a more intense steroid. They started me on 6mg 2 times per day. Cut it to 4mg 2 times per day. I am not currently using steroids since RT. I still take ER OxyContin. Most days I only take it once.

I also think my Personal Doctor was more helpful on getting to a good place with pain management than the oncology team.

That’s interesting, TA. My brother gets his first Xofigo next week, but got osteonecrosis from Zometa several years ago. Jaw is controlled with a mouthwash, and he has had 2 hip replacements for avascular necrosis. Therefore that class is not available for him. Do you have any suggestions for him, TA? He is 80 and a bit of a complainer, so I hope his first dose goes well and would like to smooth the way to avoid SE.

No, it is contraindicated because of his history.

I’m sorry that he’s in there . I’m glad you’re looking out for him . I too have the djd, but not as painful.. it’s true that old injuries or inherited traits might worsen on us..I credit your strength in holding him up now . Can you be by his side in the hospital? That’s a lot of opioids . I pray that he gets back home soon . 🙏

Yes Lulu. I have been here every day and night. Unfortunately because of the djd he has been on constant heavy pain meds ever since injury in 1995; therefore I think it’s hard to control the pain with the meds. It’s just sad that the pain has become unbearable within the last few months but all scans show nothing new. Pain in back and radiating down leg, they thought maybe a pinched nerve but nothing from CT Mylogram. He walked the block in hospital today with walker and PT. He is off Oxygen and Dilauted. Pain is still so bad. I’m scared to try and go home without the pain under control. No hospital beds hardly to be found from all the Covid

Stay strong . He needs you so . My wife by my side pulled me out of the icu . I’m Glad he’s not alone in there . Still , this is so heavy on you . Love never fails . His pain is so high that he must be there . But such pain from 95 ? That’s a lot. I have one friend here also with major pain from 95 on. These men are so tuff . You can not risk him falling now at home either . Covid is super bad on health care systems .. in normal Times they try to push people out so quickly if they can . I can imagine how it is now? They say that many thousands have skipped cancer surgeries due to the virus .. world gone crazy indeed . I appreciate you kindly letting us know how he is . Im on pins and needles for him to get a break . I’m praying for mercy🙏🙏❤️😷. Scott..

Today we seem a little too over medicated with now up to 150 Fetanyl patch, oxycodone 30/325 and cymbalta 30mg. Pain seems to now be under control by mouth so we are making progress. Praying we get to go home tomorrow. Unfortunately the oncologist performing Xofigo says he won’t get next round since he has been in hospital and will have to be reevaluated. Home health /Palliative care is now involved and states they will be coming out when we get home. What does this mean? Do they think he is dying soon?

Good morning. I’m so sorry to hear about his pain. I will have my 5th dose of Xofigo next Friday September 3rd. I have had a significant amount of bone pain in my legs, back, neck and ribs. Although at different times. For the last 6 weeks I’ve had to get a white blood count booster shot. This also produces some additional bone pain. Hydrocodone 10/325 works for me to knock the edge off and keep going. I also take a Claritin each morning it’s supposed to help. I also do the bone strengthener once a month. PSA has risen from .3 to 2.1 but the OC said that’s normal. The Alkaline Photophase has dropped from 2800 a year ago to 53 now. Hopefully that’s a good sign I can’t wait to get through all 6 and do a PET to see how it worked. I’ll definitely pray for some relief from the pain.

After the Xofigo work is done, your Dad may want to consult with a pain management specialist that is well versed in degenerative disk disease as he may find relieve by getting facet block injections with steroids and ultimately radiofrequency ablation to his hot spots. For the time being he can try adding in gel ice packs like the kind physical therapists use and a personal TENS unit. Best wishes for relief of pain and successful treatment of your Dad's cancer and degenerative disk disease.🙏
