PSA KEEPS ON GOING UP

Hi. I was operated 2002 and after five years my PSA came back . Since than it keeps on going up and now it is at 20.22 . Since i came back to Bulgaria a few years ago from Canada where i was operated things don't look good at all . The doctors here don't help much and they just keep on telling me not to worry since so many years have gone by . However i am very worried and a few days ago i went and had a SCINTIAGRAFIA done on my bones . The good news is there is no cancer in the bones. I have no idea what to do now to stop the PSA from going up even more . Can enyone help me with this ? I worry a lot ! Thank you

It appears that your only treatment to date has been A RP in 2002. I’m no expert and not a doctor but if I were you I would get a PSMA Pet/CT scan to get the best picture of where your PSA is coming from (tumors). From there you have a number of possible treatments including ADT, docetaxel, Lu177 and others.

Salvage radiation therapy would be the usual next step.

Would there not be some benefit to getting a pet scan to rule out metastasis before deciding on salvage radiation?

Maybe a bone scan. This is Bulgaria.

I just had a bone scan and there is no metastasis in them .

Radiation is usually the next treatment.

I would get a more sensitive scan such as Axumin or PSMA PET scan as 6357axbz indicated to see where the mets are. I don't think it makes sense to get radiation if the mets are spread out and outside the prostate bed. With a your PSA at 20 there's a good chance of that.

You might need to go on ADT if your mets are spread out and your PSA keeps going up.
