My husband been on Zytiga about 5 months his psa was coming down very slow last psa was 13.7 it’s now 14.5 does this means the med stopped working already? Is this something I should be concerned about? Can you have small fluctuations In a psa

Yes, that's a possibility. But, MO will probably want a trend of rising PSAs before possibly switching out the med. Maybe also a scan eg Axumin. I started Abiraterone last Jan and had a slow drop from 10.2 and over several months only got to 4.1.

Then it started going up and at one point 5.1 to 9.2 in a month

Got on Xtandi only 2 months ago and PSA still rose 😭

Gonna do a rechallenge with Taxotere

Had 15 in 2015 and a PSA nadir of 0.1 in the Summer of 2017.

I agree, that is a little confusing. However my experience has shown a few ups and downs until a very steady rise which indicated “failure”. These are my readings from just before I started Abiraterone...

It was after the 9/20 reading that I switched from Prednisone to Dexamethasone. Am back now on Prednisone and about to start the next level of treatment. Because I am Oligometastatic (2 bone mets) will try SBRT - while continuing Zoladex and Abiraterone.

Hi

I have had a similar trend with Zytiga + pred and I had SABR treatment to be met in September. My PSA has halved from 2.5 - 1.2 in four months while staying on systemic treatment. It seems to work if you are low burden cancer and oligometastic

Good for you! I am hoping for a similar result, and hopefully an ongoing reduction in PSA.

It is a small increase and may be due to irritations or inflammation. Look for trends. If the next two readings are higher, then he is failing zytiga and it's time to move to another drug.

His Oncologist has him come every 6 weeks to check his psa. Do you think that’s to long of a gap? Should it be sooner seen as though he is castrate resistant?

The Zytiga and prednisone really bothered me. I had horrible headaches. Dr. Finally put me on Erleada with no prednisone and I use a half dose and I'm now fine. .01 psa. The prednisone caused me to constantly eat. They claim this was all in my head. Right.
