IMRT delay? Significant?

I am presently in my 2nd month of ADT (Firmagon), and as a result after 1st month my PSA went from .16 to <.01 (undetectable) and T from 128 to 19. Due to the spike in Covid...and some insurance issues....instead of starting Dec. 21 (just before the holidays)....we are looking at moving it to mid to late January 2021. Anyone see any reason not to move it back? T2N1, Gleason 9, post RP (8/2019) and Decipher .78. No other sign of metastatic activity as yet (Auximin PET). We started ADT to be aggressive before hitting .2 PSA.

Since you can get firmagon almost anywhere I see no reason not to move unless your moving to the wilderness.

If you have met your insurance catastrophic limit and copays you may want to start in December since your treatments will be covered 100 percent by insurance. Otherwise see no reason not to wait.

I believe if you PSA is undetectable you could wait until the end of January. I did something similar in 2005. I was started in ADT when the PSA was 0.2. IMRT was started a few months later after I consulted in a couple of places.

Sounds good. You can be on ADT almost indefinitely before beginning RT.

How are you feeling and dealing with your treatment? Did your doctors give DECIPHER much consideration in your treatment plan?

Firmagon is a bitch right after the injection (pain for 3 days)....and the hot flashes have been fast and furious. Getting enough sleep is a challenge...and my blood pressure is running a bit high....something I have never had to deal with before. A bit lethargic....so a few pounds added as not "into" my usual work out routine. Getting a fit bit device to try to motivate activity and monitor bp a bit more (also bought a cuff to check it).

I have the exact same side effects. I started on 11/10/2020. Have you tried changing firmogon to Lupron? They offered me to take the lupron shot for 6 months two days ago when my second firmogon injection was due but I got nervous and said no, not to rock the boat. Now, I am laying in bed with the swollen from the injection site. Are you going to stick to firmogon or change it to something else?

Sticking with Firmagon for now...until after the SRT. I will know more when I get my next PSA and T reading Friday before my next injection. Doesn't make sense to change anything until after RT is done.

I am hoping the pain and swelling from injections decrease a bit each time.

IMHO. With the very low PSA achieved by the ADT, you should be ok to delay the IMRT for awhile. Personally, I would get the Covid vaccine then schedule the IMRT asap
