Sounds like trials have shown some great success stories.

Anyone have insight as to when Lu-177 may become approved/available in the US?

For ballpark planning, here's a link to the Phase 3 VISION trial, showing objectives and planned completion dates. Publications and any US FDA Approval determination would likely add some more time.

If you may be thinking of a particular site for future treatment in the USA, the trial participating locations would already have some experience with some clinical trial patients by then.

Same as the US. The data from the VISION study has to be published before it will be approved.

Hopefully there will be sufficient data to cover most of us, i.e., hormone sensitive, hormone resistant, pre and post chemo etc.

Does Lu177 kill microscopic cancer cells or only those tissues expressing PSMA?

Lu-PSMA-617 kills all cells expressing PSMA, whether microscopic or part of larger tumors.

So if no tumors are present large enough to be picked up by a psma pet/ct scan how would we know how much benefit it would have?

They have not approved the Ga68 PSMA PET/CT yet, despite of published studies and requests for the approval.

It is going to be a while longer for the Lu 177 PSMA PET/CT to be approved in the USA.

It is a shame since it seems that it is more effective in patients treated earlier before chemo is required.

Ga 68 PSMA and Lu 177 PSMA are routinely used in Europe, India, Australia, South Africa etc. Both are effective, well tolerated and have a low incidence of severe side effects in patients without extensive bone marrow infiltration.
