The normal evolution of prostate cancer makes that happen. I'd say 18 months is around average or maybe slightly better than average for stage 4. I got about a year.

Instead of going on Zytiga along with the ADT in the beginning, I did chemotherapy with ADT at the beginning. Zytiga was approved for hormone sensitive patients right after I started chemotherapy so I could not do that.
A little more than a year after starting ADT, my PSA started rising and when it went to 26, I started Zytiga. That was over 2 years ago and my PSA is undetectable now.
Castrate resistant is defined as having a rising PSA while on ADT drugs. Has he gone through all ADT drugs. If not he is not castrate resistant.
Both urologist and oncologist said he is castrate resistant because Lupron failed
Lupron has not failed. It's just not enough alone now because the cancer has learned to use smaller amounts of hormones.
My combined Chemo and ADT lasted 11 months. I started Radium 223 in Feb and my PSA started rising, my RO said it happens sometimes that PSA that rises a bit during treatment, she called me yesterday to say the PSA is now at 111 and conceded that this isn't normal. I'm not sure what they plan to do next but you could say I'm a little worried now. It did initially help with the pain but right now I'm struggling with pain and keeping food down, they will do PET scan and Xrays tomorrow to try and figure what's going on.
