Hi all, I am PC advanced stage from last 45 months aged 57 yrs. Underwent chemotherapy docetaxal in the initial stage and now from last 6 months underwent 9 cycles of cabazotaxal. But my PSA Is slowly going upwards and now at 5.15. During diagnosis it was 19 and then came down after doxeetaxal to 0.25 and after 2yrs it started climbing to .58 and to 3.1 . Abiriterone worked only for 5 months. My scan reports says as per MO showing 3 new mets somewhere I the hips or buttocks. Other images in fact has shown regression compared to last scan. Now my question is what should be the ideal treatment which can stop the progression.

I am assuming you have not had external beam radiation. That might be an option if the Mets are close together.
If the bone mets are dispersed then xofigo might be the way to go. For soft tissue Mets, firmagon or xtandi or another ADT drug.
Have you had genetic testing to see if you have a mutation that has a targeted treatment? If you had a BRCA mutation for example, you could try a PARP inhibitor. You coud try Xtandi and possibly get some time out of that.
I think your best bet is a clinical trial. A tumor biopsy and IHC can point you towards clinical trials that are right for you. If you have no visceral metastases, Xofigo + Provenge may be a good choice.
