How long can we wait for RP for a Gleason 3+4, 2/12 cores, PSA 3.94 detected in March 2020?

Hello,

My Dad was diagnosed with Grade 2, Gleason (3+4) in 2/12 cores in March 2020 (biopsy done in March but results were delayed due to COVID and we found out recently). Follow-up CT scan and MRI revealed that it is still organ contained. His PSA at the time of biopsy was 12.91 and recent PSA has dropped to 3.94.

We have decided to go for RP but will need to travel to another city (which is currently the hotspot for COVID) to get the surgery done. That is making us family nervous as it would put my Dad at additional risk.

My question is how long can we wait to get the surgery done without having the risk of it being metastasizing?

In the two cores that had pattern 4, what percent of the cancer in those cores was pattern 4? (If you got a second opinion from Epstein's lab at Johns Hopkins, it will tell you that; otherwise, it may not. The larger the amount of pattern 4, the riskier it is.

Have Epstein read your biopsy slides regardless. Then, If an RP still looks good, do it as soon as you can..I don't think Covid-19 is a factor here..If you are lucky, the hospital where your Dads surgery will be performed does not even treat Covid patients..If they do, they will be completely isolated from the other patients.. Learn this about cancer, it doesn't get better as time goes on..Best of Luck to you and dad..

Consider that no one can predict the future regarding Covid -19 infections. Indeed, you could end of waiting to a 'better time' only to discover that it wasn't a 'better time' and the process could be delayed again -- you don't need that stress. If you trust the hospital, then I would move ahead. Good luck.

The virus isn't going away, and there is no way to tell how long you can safely wait.

Posting his pathology report would help us, but the sooner the better. It's unfortunate you don't have an acceptable surgeon closer, and away from the hot spot. How far are you traveling?

You have time but I wouldn't wait too long. COVID isn't going away soon. Also sometimes the surgery pathology report comes back different than the biopsy report.

If it were I , I wouldn’t hesitate .
