Advanced Localized PCa

I'm new here, have advanced localized PCa, Gleason 9 (4/5). RP plus adjuvant tomotherapy, now on Lupron. Not in lymph nodes yet. Outlook hopeful but concerned. Anyone else have/had a similar profile? What is your current situation? Any recommendations? Thanks.

I am not too different. Had RP in Feb of last year. They did find one lymph node with cancer, all others in the area clear. Three months later that was deemed to have failed so I was put on Lupron and late this summer took 39 fractions of radiation. PSA currently undetectable, will probably stay that way until I stop ADT in about a year. Then we will see what happens....

Thanks for the reply. Sounds very similar to me - had RP in July, than began Lupron in August and had 39 tomotherapy (Accuray Radixact) treatments in September/October. PSA post-RP was undetectable in August, before starting RT and Lupron. I've been told to expect PSA for the short-term to remain undetectable due to Lupron. As you say, after taking a break from Lupron in another 8 months, then we will see what happens. Feeling pretty good, dealing with frequent hot flashes from Lupron, otherwise OK. Good luck to you!

Yeah I had the Hot Flash thing too. Am currently taking Megace for that, it cuts it down to an occasional event at night. However it's side effect is that it makes me hungry, so I have to be very careful to not gain weight. Mediocre luck on that so far.

Welcome Steve. We are in the same boat. I was G7, ECE, no lymph node or SV involvement, clear margins. I had my RP in 8/18, with persistent PSA after surgery. 6 months of ADT beginning in 3/19. 8 weeks of EBRT beginning around 5/19.

Now, I’m waiting for my testosterone to recover to see if my PSA returns. I guess we’ll be living the rest of our lives from PSA test to test.

I’ll follow you to keep up with quarterly progressions. Good luck to us!

Hi StevieR,

I am a roughly in the same boat.

Diagnosed with a PSA of 130 -140 in September 2017, Gleason Score 7b (4+3) and T3b with seminal and perineural invasion but nothing in the lymph nodes that were removed during the prostatectomy in February 2018. Five weeks after the operation my PSA had fallen to 2.07 and then after three months of Firmagon and 66 Gy of radiation to below the limit of detection where it remained for 15 months. I have had a total of 19 months of Firmagon before stopping in September this year.

I am now just over sixty days out from the last injection and some things are returning to normal, next PSA test end of December.

My initial psa was 19 in early 2018. Gleason 9. Had RP at Mayo clinic in Rochester MN March of 2018. Clear margins, SV and lymph node involvement (4). Started EBRT October 2019 for 7 weeks. Began 6 month Lupron at same time. Now I am waiting for my next PSA check to get some idea as to status. Most likely, the Lupron will need to exit my system before I really know anything. Lots of hot flashes. Mayo Clinic Urology department has been amazing in all regards.
