just finished ext beam radiation to spine now on xofigo

zytiga quite working after 6 months mo psa up to 8.13 as of 9/1 mets in bones went from 4 to 10 Mo wanted me to start xtandi I refused so he sent me to a radiation oncologist who did a scan and seen a very bad met on a vertebrate thank was causing pain he hit with ext beam 5 times pain went away. next day he started me on xofigo 10/3/19 no side effects yet. Here is my question and o will not give me any input. Are there any meds I can take while on Xofigo (6months) I'm nervous about going with no internal cancer treatment !! Please give input.

Hi,

I'll get my 4th XOFIGO in a week. I had to interrupt systemic treatment with XTANDI during this time. I currently get Lupron and XGEVA. My PSA has risen sharply from 26 at the beginning of treatment and is currently at 67. Tomorrow I have a PSMA-PET / CT to see if the increase is a flare-up or if there are other reasons. The Radium 223 IS a systemic treatment that focuses on bone mets. At some point they might want to try chemo but not on top of the Xofigo..Doubling up on the side-effects is just to risky...

I'm not sure what you mean by "no internal cancer treatment." You are still taking Eligard, right? You may need a bone-preserving agent, like Zometa or Xgeva, depending on what a Dexa scan shows. I think Provenge may potentiate the effectiveness of Xofigo.

Yes still on eligard Is Xgeva in your opion any better (less side effects) than Zometa thanks for the help.

Side effects are similar. Xgeva was slightly more effective. Zometa + Celebrex increased survival by 22%

Knowing nothing about this I did some Googling. Here's what I found:

1. Xofigo and Xtandi appear to be safe to use together. See:

2. Xofigo and Zytiga are not recommended to be used together. See:

ascopost.com/issues/novembe... However I also found another study that found no additional toxicity in taking Xofigo + Zytiga: ncbi.nlm.nih.gov/pubmed/299...

3. A phase II study of docetaxel + Xofigo was successful and showed enough benefit that the investigators have launched a phase III clinical trial:

In phase II trials the investigators are testing a small number of patients to see if a treatment is safe and to find what doses are safe. In phase III they test a large number of patients to get useful statistics on how effective the treatment is.

4. The FDA drug label for Xofigo says that the company did not study drug interactions and has only very limited information about that. However the label does say that androgen deprivation therapy was used during all of the testing. See:

So, as far as I found, it should be safe to take enzalutamide (Xtandi) or docetaxel (Taxotere) and other drugs also appear to be safe. However, check this out with your oncologist who may (or may not!) know more about this.

My husband is beginning Xofigo treatment tomorrow. His oncologist told him that Xtandi and Xofigo taken together has led to an increase in the number of fractures in the long bones. He said that they do not understand why this is the case but my husband has decided not to take them together.

I stand to be corrected! The studies showing an increase in fractures was the combination of Zytiga and Xofigo, not Xtandi and Xofigo. Our oncologist suggested that because Zytiga and Xtandi are similar drugs there may be an increase in fractures as well using Xtandi and Xofigo together. Sorry for the confusion.

Exactly the same reason I received from my doctors. For safety reasons, XTANDI should no longer be administered simultaneously with XOFIGO.

Hi,

Good to hear you are not having side effects after starting Xofigo. In will be starting is in a few weeks and am concerned about the side effects. Recently stopped Xtandi because of extreme reaction to it. After 3 months I still have problems with no feeling in my feet/toes. Was on Zytiga before and it stopped working. Still on Eligard and Xgeva monthly shots.
