Anyone’s PSA go up after Jevtana?

My husband went for his 2nd Jevtana infusion today, and his PSA went from 186 to 400 in the 3 weeks since! The doctor said it’s possible that the PSA could be shedding into the bloodstream, hence the jump. Has anyone heard of this? He did go ahead with the 2nd one today. I’m just really not sure what to think and would appreciate any comments or information anyone might have.

Yep - it may be a sign it's working well. You have to look at the PSA pattern over time and any scans.

T-A is correct...When you are under treatment the numbers can bounce around . There are lots of things going on..Don't become a PSA slave...But have plan 2 ready if plan one fails..

My doctor put me on Jevtana after taxoture. The Jevtana never did work. After four treatments of the Jevtana where my PSA was doubling every 3 weeks, the doctor stopped the chemo. We are now looking at other options including Lu177-psma or Xofigo. I just met with a doctor at the Honor Clinic in Scottsdale yesterday trying to get enrolled in the Clinical Trial for Lu177. I don't know what will happen with this yet. It appears that probably most of the 750 slots taken, since our appointment with one doctor was canceled because they had slots.

If the Chemo isn't working there are other options such as Zytiga or Xtandi that you could try. I got 22 months out of Zytiga before it stopped working.

Since he has already done his second infusion, wait for about 15/20 days (not more than a month) and then test his PSA again. If it has jumped further from 400 to say 450/500/600, get a PSMA PET-CT scan done immediately. Even if you have to pay for it yourself. This scan will tell you FOR CERTAIN whether the disease has progressed or regressed and by how much. If the disease has actually regressed, then it may be OK to go for another infusion. Otherwise, just stop Jevtana immediately. If your doctor insists on another infusion, dump him and find another.

My husband had the same initial reaction and frankly after 6 cycles his PSA has bounced around. His MO did say the same thing about the cells dying off etc. After the first one he did have a huge jump then it went way down and is still bouncing around.

But clinically he has improved dramatically so important to look at the total picture.
