I have had the TULSA PRO type of HIFU as salvage treatment after failed radiation. It differs from traditional HIFU in that it delivrred via the urethera as opposed to rectally. There is a rectal component as well but it is a cooling device that is intended fo prevent heat damage to the recatal wall. All this is done with real time imaging to monitor temperature in the ablation area.

This particular procedure, in my case, required an overnight stay in hospital and a catheter for a few days. Very little side effects after that. I am pleased with the results but keep in mind that my therapy was salvage whole gland instead of primary treatment.

Not sure if your Tx was recent or near the time of this gentleman's initial post over a year ago..... Interested in your experience with HIFU as I am also a failed radiation patient..... How did they determine that you were a good candidate for Salvage ( Biopsy of what remained of your prostate? Low PSA and doubling time?? Proven to be localized to the prostate after MRI/PET.... Have your results held up??? Side Effects from the Tx?? Where did you have this done?? Thanks for anything you care to share...

My Tulsa was in March 2019. I found it on clinical trials.gov

The trial has several arms, one of which covers treatment after failed radiation.

The inclusionary and exclusionary criterion are listed in the clinical trial site.

Just to be clear...I tried to be in the trial but due to mets, could not be. They treated me as a private patient with the same techniques but at my expense.

I'm curious as to why you would choose HIFU if you had mets?? Did they treat the mets with the HIFU as well or are you using another alternative to address the mets

The hifu was treating recurrance in the prostate after failed radiation. The mets were treated with SBRT.

Thank you for the clarification...... I guess the mets could not be reached with HIFU? OR... can it not be used on anything but soft tissues??

HIFU is only used on the prostate. The type I had is TUSLA PRO, which is done via the uethera. My mets were treated with stereotatic radiation in the US.

Thanks again for responding..... there seems to be an ongoing controversy re: how effective Radiotherapy is for residual mets.... but I think that has more to do with soft tissue than bone.... I'm glad you are making progress against this disease...

My husband just had HIFU in Germany (April 11th) and is very happy with the results so far. No adverse side effects. He will have his first PSA on July 11th. You may find this publication useful. It was written by the doctor who performed his treatment. The doctor has been performing HIFU treatments for 23 years. The TURP took 35 minutes and the HIFU took 46 minutes. The cost to us was $6,3045 for TURP plus HIFU done on the same day. We have submitted the amount we paid to United Health Care Ins Co. We will see if we get any reimbursement. Hi Jhon I will be 65 in December, was diagnosed in 2014(Gleason 9) had my prostate removed 2015, 2016 did 38 rounds of radiation, this year did an Axumin scan found small metastasis in my lymph glands, 8 round of radiation and lupron.

HIFU can be effective particularly if the cancer is defined and not multifocal. The best doctor for HIFU in the US is in Florida.

You should also consider FLA, focal laser ablation. Texas is one state where it is done. FLA is a newer treatment. The doctors are getting better and the results achieved by the best of them are very good. FLA is said to be more accurate by quite a bit by those I have spoken with who have had FLA and really gone in depth researching the procedures.

Both treatments have similarities. HIFU may have recently started to get a technology upgrade to catch up with FLA. HIFU has a longer history.

Insist on Rocephin or Cefdinir to be used to prevent sepsis during any biopsy or ablation procedures rather than fluoroquinolones which can have many side effects including an aortic aneurism. That sounds like fun huh? It can happen many months after the drug was prescribed.
