I had HIFU procedure July 2018. My PSA was a 7.3, but After a drastic diet changed, I lowered it to a 5.3. I had a Gleason 7. After 6 weeks I have pain trying to urinate. I still have subra cartherter in. Doctor may use an endoscope to see if I have any blockage. Some days I have a good , slow stream, some days a very painful dribble.

I will follow up after I see the doctor.

Sorry you are having such symptoms. Sounds like you may need a urologist who can remove prostate tissue so that you can urinate well. That suprapubic catheter makes no sense. Are you in the U S ?

I've had a supra pubic Cather for 8 months now! One a month they change at the urolgyst , they say the blockage is due to a stricture in my uretha due to trauma, the only trauma I can think of is riding on a dirt road, which once had a train rail on it for 8 hours, when I came home had BPH. What I do every night is disconnect the bag plug up the connector hose and drink a lot of fluids, and pea through my penis, after about a month of doing this I can now empty about 80% of my bladder, it's a struggle, but once it starts to flow, it's w orth it, every night it seems to get easier. The urolgyst never told me to do this and when I asked him about a TURP he said it would just close up again due to scar tissue, I don't believe him think it's just money making thing, I havestage 4 metastasized , on lupron, prednisone, and zytiga. Miss swimming in the ocean, talked to a friend friend who a dr. And said I could remove the catheter w ith 50cc syringe and put a waterproof patch over the opening and go swimming, that's my next challenge, don't want to spend the rest of my life with this SP catheter, I think the longer you have it in your bladder, the harder it is to get back to normal. When all you get is a dribble ,turn around and sit down for 15 seconds .Stand an try again .works every time for me ,and I have had P.C. for twelve years .M.J.

When I did my HIFU, I had the catheter removed after 16 days. I was peeing between 100-120 mL with it closed. After 2 months normal, I developed a urethral stricture from debris and required a TURP to clean it out. Now 5 weeks post-TURP, can pee with decent stream but not feel empty, sometimes having to go 3 times in 20 minutes. Follow up exam tomorrow. Incontinence is bad, especially walking or standing up from sitting position.

I want to think every one for your comments. I am going to the doctor today. Stream is better. Something else I did not mention. My testicles swelled really bad. First, it was the right one, then the left one. Its been two weeks since the left one swelled, and it's not quite back to normal.

My HIFU was late July. Needed supra pubic catheter for 5 weeks. Stream is better now...not perfect and occasional blood still comes out. Erectile function is ‘meh’...it works but not robust. Wondering how much I should be expecting PSA to be going down and by when?

The expected PSA will depend on whether you had whole gland HIFU, hemigland HIFU, or focal HIFU. Because it is still experimental, no one knows what to expect or what is prognostic for success. It probably will be different depending on lots of factors (field of treatment, initial PSA, size of index lesion, cancer outside of index lesion, grade, etc.)

I had a partial focal ablation (HIFU) Sept 2018 after finding a significant lesion on the 3TMRI (anterior)..Surgery was 2 1/2 hours....I had a catheter for 7 days and had a few challenges with regard to incontinence (went to physical therapist who specialized in pelvic floor strengthening)...14 months later....PSA .6, no erectile dysfunction, minimal incontinence..Dr Stephen Scionti was the urologist....Very happy with results
