I went with the Xgeva. I was very hesitant like yourself as my main concern was the osteonecrosis of the jaw side effect. Get a dental work-up and make corrections to your teeth now if you decide to take the medication, and then take care of your teeth. Taking those algae calcium products will be what you do as a supplement as you’ll need to replace the serum calcium that the Xgeva draws on to strengthen your bones. I take calcium 600 MG pills twice daily along with Vit D3. Have a fatty snack or meal with those algae calcium products as it will better absorb into your body. I’ve had 5 injections of Xgeva so far and will take it for a full year. I’ve had no side effects to date and I am tolerating it well. Peace and good luck!
In addition to prevention of fractures and spinal compression, it also has a potential survival benefit - a possible 22% increase in survival for men with metastases, at least that's true for Zometa+Celebrex:

Of course, check calcium and Vitamin D blood levels and supplement only if necessary.

If you are looking for an alternative to calcium algae products you can try estradiol patches to “add-back” low-dose transdermal estrogen for bone health:

At least fewer side effects than Xgeva and should reduce hot flushes too. A friend of mine uses these patches to help with his triple blockade (ADT3).
My husband has been on Xgeva for close to a year - no side effects whatsoever. His MO has him taking Calcium, D3 and fish collagen (the latter apparently keeps the bones from getting brittle).
I have been getting xgeva shots for the last couple of years. On problems. Don't worry about it.
I am on Xgeva. Yes, I have experienced some side effects. I had reservations about it as well. I have already broken and fractured bones. So, I hope it will do what it's intended for. The dental stuff DOES scare me
I'm Xgeva 14 months now no side effects and injection is very minor.i also had extensive dental work done while starting it no I'll effects.
The only side effect I got from zometa is a fluy feeling for a day and a slight temperature. As far as my teeth are concerned, I got all root canals and fillings
done beforehand. The necrosis of the jaw business I thought was kinda rare. I get a yearly dose of zometa. Denusamab or Prolia does the thing but doesn't go through the kidneys.
