Hi everyone l have Mets to my bones and lymph nodes. Been stable for a few months on xtandi. PSA rising 9-15-22-25-30. Have a Brica2 mutation. Talked to UCSF going into off label trialwith parp inhibitor Olaparib or rucparib?.
Came back and went to my local OC for my Xcheva shot. He wants to start radium treatments immediately. This would exclude me from the parp trial. His reasoning is that radium will significantly improve my OS. He feels the trials may or may not work and by the time you find out the cancer may spread to soft tissue. And you can not exercise the option
I feel like a mouse in a maze. I would appreciate any information on both sides. I like everyone else is interested in my quality of life and survival time.

Unfortunately, treatment with enzalutamide is an exclusion for this trial.
As far as I know, you shouldn't have to go on a trial to get treatment for BRCA2 with a PARP inhibitor. You should be able to get that now or later.
Has your doctor talked to you about chemotherapy? I know a lot of people, including myself at one time are afraid of it, but most people tolerate the side effects well. Many guys on this forum have taken 6 cycles of Docetaxel. I did it once and will do it again if it can help. Response with Docetaxel varies for those with the BRCA 2 mutation. Generally it's low, but platinum-based chemotherapies are more likely to work with BRCA mutations. Just something to discuss.
As far as Radium goes, the argument your doctor is making for doing it now is valid. Visceral mets are an exclusion for Radium so the longer you wait, the more likely it is you will develop them and be excluded. If you can get on a trial and do both, that's great.
I'd discuss all of these with my doctor. Just my opinion.
