Disappointing to see such a small reduction in insulin resistance (4%), but given the inexorable effect of ADT on most men, it seems like a big achievement when compared to the 36% increase in the control arm.

Abstract

PURPOSE:

The objective of this study was to test a low-carbohydrate diet (LCD) plus walking to reduce androgen deprivation therapy (ADT)-induced metabolic disturbances.

MATERIALS AND METHODS:

This randomized multi-center trial of prostate cancer (PCa) patients initiating ADT was designed to compare an LCD (≤20g carbohydrate/day) plus walking (≥30 min for ≥5 days/week) intervention vs. control advised to maintain usual diet and exercise patterns. Primary outcome was change in insulin resistance by homeostatic model assessment at 6 months. To detect 20% reduction in insulin resistance, 100 men were required. The study was stopped early after randomizing 42 men due to slow accrual. Secondary outcomes included weight, body composition, lipids, and prostate-specific antigen (PSA). Changes from baseline were compared between arms using rank-sum tests.

RESULTS:

At 6 months, LCD/walking reduced insulin resistance by 4% vs. 36% increase in control (p = 0.13). At 3 months, vs. control, LCD/walking arm significantly lost weight (7.8kg; p<0.001), improved insulin resistance (↑36%; p = 0.015), hemoglobin A1c (↓3.3%; p = 0.01), high-density lipoprotein (HDL) (↑13%; p = 0.004), and triglyceride (↓37%; p = 0.036). At 6 months, weight loss (10.6kg; p<0.001) and HDL (↑27%; p = 0.003) remained significant. LCD/walking preserved total body bone mineral count (p = 0.025), reduced fat mass (p = 0.002), lean mass (p = 0.036), and percent body fat (p = 0.004). There were no differences in PSA. Limitations include the effect of LCD, weight loss vs. walking instruction are indistinguishable, and small sample size.

CONCLUSIONS:

In an underpowered study, LCD/walking did not improve insulin sensitivity at 6 months. Given most secondary outcomes were improved at 3 months with some remaining improved at 6 months and a secondary analysis showed that LCD/walking reduced insulin resistance over the study, supporting future larger studies of LCD/walking intervention to reduce ADT-induced disturbances.

Thanks for posting this info, Patrick...I walk 5K about 50 minutes a day and lift weights, but 20G of carbs is too much for me....about 100-150 per day for me....Broccoli and my blueberries, plus V8 juice do me in....thinking of letting V8 go and just doing tomatoes will help...I love me blueberries ...

I've gone low carb since DX in 01/2015 and still do IF(Intermittent Fasting) from 9pm to 1pm next day. I now take Metformin before I break fast (per a previous post) and still am out on the roads run/walking.

Exercise may be an anti-depressant:

Thanks Randy...Getting to 20 gm carbs per day seems almost impossible, but I am working on it...also going to ask MO to put me on metformin.....thanks for posting this info...

I do IF as well....no eating from 7 or 8 pm to 1 pm the next day....water only or tea...

Unlike most humans, who are descended from ape like primates that lived on fruit in trees, I believe that I am descended from grazing animals. I love bread, pasta, crackers, rice and corn. My wife is always trying to get me to eat more fruits and vegetables and lay off the carbs, but I munch carbs when she isn't looking.

I know I'm going to pay for this behavior. Is there a Carbohydrates Anonymous organization I can join?

I suspect that the low carb/walking arm is really a low processed carb/walking arm and that the issue is not total carbs but processed (quickly digested) carbs...IMO of course.

I was thinking the same thing. Going to see if there was any info if they kept track of what the carbs were.

Sadly, this reminded me to walk to the refrigerator and check on my steak. Unlabeled, but by the feel it is a one pound rib eye. Descended from hunters no doubt. Always wanted to be a vegetarian. Will be sucking up broccoli soup through a straw one day. So sure I will get my wish. Rain has stopped, sunny but cold day. High 60 degrees. Great grilling weather! And because it tics off everyone I know, and by extension probably most everyone in the known world: "Go Tom Brady, go!" Old men should be recognized.

This is good info, thanks. I'm on strict no-starch diet for over 15 years due to autoimmune condition (ankylosing spondylitis). With this diet I was able to slow down progression and put the disease into remission with only occasional flare ups caused by mostly stress or diet deviations (usually unintentional, like not checking unfamiliar foods with iodine for traces of starch). I hope that this play a positive role in my current treatment. Over one year on ADT and I still have normal blood sugar, actually lost some weight (I'm 6' 180lbs now, comparing to 195 before Dx) and feel pretty good overall.
