How do I interpret PSA upload?

Treated with Zytiga for 13 months. During my first 11 months the PSA has followed a decline in each reading. from 1350 to 0.17. But in the 12 month reading the PSA went to 0.19 and in the last reading of the month 13 to 0.26.

My doctor says I do not have to worry. But I want to know when it is interpreted that Zytiga is failing to work.

On the other hand, my Alkaline Phosphatase continues to fall in each reading.

It's a slow rise. It's not doubling. So is good for now.

PSA can bounce up and down slightly on Zytiga, it happened to my father as well.

Follow your oncologist advice and try to not worry!

Thank you very much for the reply. I've heard about the rebound effect. I do not know what the acceptable oscillation is.

I will wait for future results to see a rebound effect or trend.

I think it's a good sign that your ALK PHOS is still going down. And also the rise seems to be slow. Looks to me like you have some time. I would keep going with Zytiga for now.

What is your current Alkaline Phosphatase level?

That is a fantastic response, there will be ups and downs in your psa, certainly more than the .1 you are having now, It is important to ride these ups and downs and get the most out of all these therapies. Some have a second response to zytiga by using dexamethasone as a steroid instead of prednisone, but you would certainly wait for significant consistant rises before doing this. I wish you the best

According to the latest criteria for PSA progression in research studies, PSA must reach a minimum of 1.0 ng/ml. But PSA alone is not enough -- there must also be signs of RADIOGRAPHIC progression before the therapy is declared failing. You are far from that.

What does RADIOGRAPHIC progression mean, In 3 years my psa has gone from 0.8 to 1.3 and back to 0.8.

Hello! Did this happen to you without changing the treatment at all? Have you been during these 3 years in Zytiga?

Only on Eligard, psa will not go any lower.

It means that the bone scan/CT or other imaging shows spread of the cancer

Joancarles, from 1350 I’d say that’s some drop . We all walk this dammed PSA phychological tight rope. Eat well stay moving And recovery shall come.. Stay 💪
