After months of blunders by my dad’s oncologist, we finally received the results of his Guardant testing.
He has high MYC amplification and moderate RAF1 amplification.
I am struggling to understand what this means and what the next step is. We have a consultation next week with Prostate Oncology in California.
I am attaching the tumor response map as I’m not sure of its relevance.
Also is it of any significance at this point to see if the AR-V7 gene is activated? Is that a moot point since he’s progressed on all traditional treatment?

Frustrating! But glad you got results so you can move forward. It’s Greek to me but we’re trying to get the test also. Last visit they didn’t have a clue what I was talking about. Who are you seeing in CA?
You found out what the problem is with much genetic testing - there is little to be done about it. There are only 3 therapies that are specific to certain specific somatic mutations, platins, PARP inhibitors, and Keytruda. If the mutations aren't specific for one of those, and the ones you mentioned aren't, there's nothing that can be done that is tailored to them.
As for the AR-V7, you already knew that due to early failure of Zytiga and Xtandi. In some cases, it can be reversed by chemo or by supraphysiologic doses of testosterone (in men without symptoms).
