Avodart Offers No Benefit During Off Phase of IADT
Gus,
The issue with Avodart during the on-period is whether DHT levels justify its use. It isn't going to help if DHT is low. Dr Myers, I believe, aims for <5 ng/mL. The study used "ng/dL", sodivide by 100.
Look at Fig 2 (DHT) [1]. Note that both groups went into the off-treatment period with DHT ~3 ng/mL. Meaning that the placebo arm did not need Dutasteride anyway. & perhaps no-one in the other arm did either.
Notice that DHT increased in both arms as the months went by. Avodart was seemingly unable to keep DHT under 5 ng/mL in the off-period. The graph is similar for both arms - so the results should be similar.
Contrast with the Finasteride study:
"Finasteride was administered in the off-treatment interval only, in contrast to this present study."
"Scholz and colleagues showed that finasteride increased the time off treatment from 15 to 31 months."
Why was 0.5 mg Avodart ineffective in the off-period?
Here is an oddity:
"We found that 23% of all patients had prior ADT"
Design fault? Prior ADT periods affect subsequent effectiveness.

In a couple of places the study referred to the effect of 5ARIs as "inhibiting" or "masking" PSA secretion. Myers also disagrees w/ this; he maintains that the PSA reduction on Avodart is real - that it reflects disease reduction. A minority view.
So, how do we access the results. I clicked the link, went to the study site, then what?
Thanks,
Craig
