So my 5 year old has been on Tranquilin 2.5mg twice a day for the last 3 weeks. We had a review today and they were asking me questions and I could not answer as he is at school when meds are onboard. When he has been with me I don't notice any difference. School says he is a little more settled but that's about it. should I be seeing more improvement by now??? or do they need more time? When I pick him up from after school club he is in a really bad mood. Should I be asking about a slightly higher dose? Am I asking for too much? We just all need some calm and summer hols are coming.

In general a week is usually enough time to see a difference with immediate release methylphenidate. He may need a higher dose. Does he have autism in addition to adhd? Only asking because of your name:). Because kids with autism have less predictable improvements on ADHD medication in general than kids without autism who have adhd. However, he may just need a dosage increase. Usually after a week you can tell if an immediate release stimulant at that dose is sufficient. The non-stimulants can require a longer period to see an effect & judge the impact of a dose adjustment too. Good luck!
PS Not sure if his bad mood after school is new since starting medicine? Some kids with ASD work so hard to make it through the school setting, they’re just wiped by the end of the day. Finally at 12 our son could explain he needs some “minimal social interaction downtime” afterwards to recover.
Thanks for your great question. We were told that medication, when it is the right dose, type and correct time should control about 60% of the symptoms from ADHD. Since your son is on a stimulant, it should help control excessive talkativeness, impulsive behavior and should allow him to better control him self.
There are some symptoms it will not control: focus ( unless related to impulsive behavior) and mood.
You are not asking to much when you need better control, you will see a difference.
