I would get a second opinion on the MRI. Pirads 3 puts those two areas in question. The low psa sometimes can be aggressive cancer. You might want a complete second opinion on your entire case. Remember, surgeons recommend cutting, radiation oncologists recommend radiation and med oncologists recommend drugs. If you find one of those who recommends otherwise, they are being totally honest.
My uro doesnt do RP surgery and recommended AS after my biopsy. One core G6, two cores suspicious, PSA steady after a year at 5.3. MRI shows similar results and pirads 2.
ihaven't heard that a low psa is a sign of aggressive cancer
Unfortunately, some men with G9 or 10 have had low psa results in the 2 to 2.5 range. I think in these cases they either have symptoms or the doc feels bumps when doing the dre which then leads to a biopsy. That's one of many reasons why psa tests alone are not always indicators of cancer.
Many men with BPH have high psa counts until the get treated for that. My uro told me about a patient with super high psa who then had 750 grams of prostate removed and still had high psa from the remaining prostate tissue.
It's almost like an inexact science. There's so many factors and nothing is really definitive. Biopsies done with ultra sound only cover about 1% of the prostate. Even those done with mri fusion can miss small tumors occasionally. Pirads scores aren't very accurate either. Lots of guys with pirads 4 or 5 have come back negative for cancer after biopsy.
Good luck with your issues.
k thanks, do you think its unusual for a urologist to question me if i wanted a 2 opinion on my MRI ?
If he's an up and up player, he should have no problem with it. I think some docs think you dont trust them when you ask for second opinion. Remember, if you ared sending MRI or lab samples for 2nd opinion, it's those folks not the doc who are being checked on.
Remember, get opinions from each type of treatment giver, read oncologist, surgeon and med oncologist if you do end up with cancer. Research those treatments and understand the potential side effects of each. Decide on what you are comfortable with, if any (active surveillance may also be an option). Know the effectiveness of each; radiation and surgery have close to the same survival rate so the possible side effects may play into the decision. Once you decide and go through with it, dont look back, accept what is and carry on with your life. Remorse over the decision won't help and you won't be able to change it anyway.
