Wake up shaking - but not visibly

I wake up vibrating most mornings and it is getting worse. I just had bloods done and I am not converting enough T3 from the results, I do have an appointment with an Endo this week from The Thyroid Clinic but is this something I should mention - is it something I should expect? Is it a result of low T3? or something else?

Regarding Vit D I am commencing 8,000IU for 8 weeks, 6,000IU for 8 weeks, 4,000IU for 8 weeks and then 2,000IU as per the above clinic recommendations.

Was test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

How much levothyroxine are you taking

Do you always get same brand levothyroxine at each prescription

What’s the range on B12 …..if it’s a serum test it’s very low

Was this Serum B12 NHS test?

Or Active B12 private test

Hi there - the test was 9am 24 hours after last dose and no food prior.

Test was done privately via Thyroid Clinic UK (thethyroidclinic.co.uk/) They did not provide ranges but I have asked for them.

I am taking 100ug Levo, brand is Euthyrox by Merck (I moved to UK from UAE and still on same medicines). I have not taken any other brand.

I am not sure what B12 test they did, I have asked them to clarify

I did an Active B12 in May as part of my wellbeing check up and it was 103pmol/L (range 25.1 - 165.0). However those bloods were taken as part of a wider check up and not 24 hours after lost dose.

I am speaking to the clinic on Wednesday and will ask.

I did folate as part of my wellness check-up too. It was 4.7ug/L and ideal is >2.9 but also perhaps not at the ideal time.

That’s low

B12 is most likely serum B12 …so also low

Ideally you should have test for Pernicious Anaemia before starting on B12 …..but it’s an expensive test

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need separate methyl folate couple times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 until over 500

Can I refer you back to my reply a year ago - as that pretty much details all the treatment options you may be able to consider going forward with this endocrinologist on Wednesday.

Did you ever get your T3/T4 thyroid blood test results - pre- thyroidectomy - so have an idea where in the ranges your T3 / T4 need to be sitting for you to be feeling well ?

Your T3 is too low for you to function well - this in turn means you are not metabolising and likely struggling to extract key nutrients from your food, irrespective of how clean and well you eat -

and low levels of key nutrients - ferritin, folate B12 and vitamin D can compound your ill health further -

your ferritin looks ok - is B12 active / serum ?? - vitamin D low and we have no folate reading.

For reference as a woman - I aim to maintain my ferritin at around 100 - folate 20 - active B12 around 125 ( serum B12 500++ ) and vitamin D at around 125.

and which are also needed to be maintained at optimal levels for any thyroid hormone replacement to work well and be utilised and converted into T3 in the body.
