What happens if you move county in UK with a shared care agreement for T3 in place?

following on from my previous post, it looks as if I am about to get a shared cared agreement between my Endo and GP for them to prescribe T3. Fingers crossed!

I just wondered, if all goes well and a few years down the line, what happens to my prescription if I want to move.? (Specifically 200miles away to Kernow-Cornwall)? Hypothetical. A dream I have.

???

Do I have to start again with a GP begging them to listen to me and refer me to an endo and start from scratch?

Will a new GP automatically refer me to new, local Endo to query if I actually need T3?

Do I go back to the original hospital that is prescribing and hopefully can continue, even though they will be in a different county, until I find a GP who is willing to take on a shared agreement?

Does a hospital down there have to prescribe instead of mine?

anyone had any experiences with Cornwall and T3? (Please PM me)
