Could someone check the attached results and let me know if I should be on medication? I've been having issues with energy levels, motivation, some anxiety and weight fluctuations.
This has been ongoing for years, I was told roughly 6 years ago I was borderline underactive however I was never treated. I went to the doctors last week.i was told my thyroid is up and down over the years after she looked at previous results, they weren't very helpful so I had a private test.
My grandad had a partial thyroid removal and my mum has had hers completely removed due to thyroid cancer.

With that TSH and your symptoms, you certainly sound hypo, but T4 is quite high. Would be a good idea to get your thyroid antibodies tested - if you have Hashimoto's/autoimmune thyroid disease, your levels can fluctuate a lot in the earlier stages especially because periodic attacks on the thyroid cause excess hormone to be dumped into the bloodstream. So your high T4 could be due to that, and it can be a bit deceptive because you get varying highs and lows. It might help to also post previous test results of you have them, and then people can get a better idea.
Would definitely not start taking levothyroxine until you have antibodies etc tested because that could push your T4 up even more, which wouldn't be good. I think T4 levels can vary slightly through the day as well, so time of testing matters (early morning better).
Not necessarily - you ideally need both kinds of antibodies checked, both TPOAb (what you've had) and TgAb (thyroglobulin). Some people only have one set of antibodies show up, and even if they're low, it doesn't necessarily rule out autoimmunity because it can fluctuate.
Have you had vitamin and mineral testing done recently as well? It might be worth getting a full thyroid panel with both types of antibodies and key vitamins and minerals to get a better pictof what's going on for you. Others will hopefully come along to comment too, but again would be very cautious about taking levothyroxine yet because it could send your T4 levels over range, and that can be a risky game that wouldn't benefit you. Best thing to do would be full testing in a few weeks or more probably, making sure it's early in the morning.

get FULL thyroid testing including BOTH antibodies yourself
Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)
On T3 - day before test split T3 as 2 or 3 smaller doses spread through the day with last dose 8-12 hours before test
