Although you need to start on 25mcg levothyroxine because of your age …..you still need to increase dose as fast as tolerated

Bloods should be retested 6-8 weeks after each dose increase

ALWAYS test thyroid levels early morning and last dose levothyroxine 24 hours before test

Typically dose is increased slowly upwards in 25mcg steps over 6-12 months

Unless extremely petite likely to eventually need to be on at least 100mcg levothyroxine per day

Starting on very low dose results in becoming more hypothyroid

Symptoms of hypothyroidism

thyroiduk.org/wp-content/up...

Levothyroxine doesn’t “top up” failing thyroid…..it replaces it so it’s important to increase dose as fast as tolerated

Standard STARTER dose is 50mcg …..but starting on 25mcg for first 6 weeks if over 65 years old

ESSENTIAL to test vitamin D, folate, ferritin and B12 too

Extremely common to need to supplement to improve low vitamin levels

optimal vitamin levels helps tolerate increasing dose levothyroxine

Have you had thyroid antibodies tested for autoimmune thyroid disease

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

How low is vitamin D

What dose vitamin D are you now taking

B12 also too low

Have you had folate and ferritin levels tested

How long have you been on 25mcg levothyroxine

Get bloods tested 6-8 weeks after any dose or brand change in levothyroxine

ALWAYS take levothyroxine on empty stomach and then nothing apart from water for at least an hour after

Many people take Levothyroxine soon after waking, but it may be more convenient and perhaps more effective taken at bedtime

Are you taking levothyroxine waking up or bedtime?

You could try cutting tablet in half and take half waking and half at bedtime

No other medication or supplements at same as Levothyroxine, leave at least 2 hour gap.

Some like iron, calcium, magnesium, HRT, omeprazole or vitamin D should be four hours away

(Time gap doesn't apply to Vitamin D mouth spray)

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

Similarly if normally splitting your levothyroxine, take whole daily dose 24 hours before test

which brand of levothyroxine are you taking

Try taking levothyroxine at bedtime

Vitamin D …..is that 11nmol or 11ng/mL

In Cyprus it’s probably 11ng/mL

In U.K. vitamin D tested in nmol

Aiming for vitamin D around 100nmol …..U.K. units

That’s 40ng/mL EU units

endmemo.com/medical/unitcon...

Remember Vitamin D supplements should be at least 4 hours away from levothyroxine

Important to supplement magnesium when on vitamin D and especially when very low vitamin D

Magnesium supplements should be afternoon or evening as relaxing and helps sleep. But magnesium must be at least 4 hours away from levothyroxine

Web links about taking important cofactors - magnesium and Vit K2-MK7

Magnesium best taken in the afternoon or evening, but must be four hours away from levothyroxine

Vitamin K2 mk7

Stomach bloating, acid reflux and gluten intolerance all extremely common HYPOTHYROID symptoms and should improve as dose levothyroxine is increased and vitamin levels are improved

Breathlessness often low iron/ferritin

“Air hunger” can be hypothyroid symptom and again will improve as dose levothyroxine is increased

Only add one supplement at a time

Starting with vitamin D

Wait at least 10-14 days to assess any improvements or problems……before adding another supplement

B12 and folate frequently low when hypothyroid and if vitamin D is low

Low B12 symptoms

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and then add a separate vitamin B Complex after two weeks

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

B12 range in U.K. is too wide

Interesting that in this research B12 below 400 is considered inadequate

healthunlocked.com/thyroidu...

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid). This can help keep all B vitamins in balance and will help improve B12 levels too

Difference between folate and folic acid

healthline.com/nutrition/fo...

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules.

(You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Other Vitamin B complex options

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endo.confex.com/endo/2016en...

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement and …..continue separate B12 until B12 is over 500

Keep going with the euthyrox. I've been keeping a food diary and also added my energy levels per day in it. Also be careful with what you are eating as it does effect your levels. DO not skip any meals, in the beginning I also had the dizzy spells and nauseous . It stopped after I controlled my glucose levels. Don't have any side effects anymore. I can't say if this will work for you, just saying what I've been experiencing. Hope this is helpful
