hello,

I am new here and looking for some advice.

I had my TSH result back yesterday and it was 4.05, which my doctors feel is in the normal range.

I have been told by a private IVF clinic that they would like my TSH to be 2.5 before commencing treatment. I have previously had 3 miscarriages and feel that my thyroid function could be a contributing factor to this.

Am I eligible to start any treatment and how would I approach this with my GP?

My mum and both grandparents have thyroid problems too.

Of cours a TSH of 4.05 is in the so-called 'normal' range. You don't have to be a doctor to see that. But it's far from 'normal'. A 'normal' (euthyroid) TSH would be around 1, never over 2 and you're hypo when it gets to 3. So, it's more than likely that your thyroid function - or lack of it - is what is contributing to your fertility problems. Your GP should know that.

I really don't know how to talk to doctors, they're a funny breed. You can never tell how they're going to react. But, try telling him that you want to conceive and your clinic wants your TSH down to below 2.5 before starting treatment - which is a perfectly normal request. Or, better still, get the clinic to write to him.

Was this test done early morning, ideally around 9am (this gives highest TSH )

Clearly TSH is suggesting thyroid problem

You need FULL Thyroid and vitamin testing done

NHS should retest 6-8 weeks after first abnormal test

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12 at least once

with autoimmune thyroid disease in your family you must have thyroid antibodies tested

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies 

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. 

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis 

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Guidelines are a bit ambiguous

It says TSH should be below 2.5 pre conception…..but it’s not clear if that’s ONLY for patients already on levothyroxine. Frequently that’s how GP’s interpret it
