Advice Please

Ferritin 19ug (10.0-204)

Folate 3.8 (3.1-20.5)

Both normal but low

Do I need to take any action to increase Ferritin and Folate please?

B12 seems ok 562ng/L(187.0-883)

Going to test Active B12

Thank you any advice is much appreciated

Ferritin is deficient

Are you vegetarian or vegan

Pre menopause? Heavy periods?

GP should be doing full iron panel test for anaemia before (likely) prescription for iron supplements

It is possible to have low ferritin and high iron, so you need full iron panel test

In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron

What are your thyroid results and ranges

How much levothyroxine are you currently taking

If going to test Active B12 do so before starting on vitamin B complex

Hi SlowDragon Thank you for the informationI am currently waiting for a colonoscopy I have gastro issues including bleeding from back passage.

I have had a positive FIT stool test and Calprotectin stool test 156.

I am currently taking 50mcg Levothyroxine 20mcg T3 Liothyronine

My last thyroid tests

T4 10.07(9.0 -19)

T3 4.4(2.4-6)

TSH 0.01(0.35-4.94)

I also have Secondary Adrenal Insufficiency.

I used to Take 100mcg T4

4 days & 75mcg 3 days before starting T3. I am definitely better on T3 but very tired in afternoons.

Was this test done as recommended

Testing early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

Day before test splitting T3 as 3 smaller doses spread through the day, with last 5mcg dose T3 approx 8-12 hours before test

FT4: 10.7 pmol/l (Range 9 - 19)

Ft4 extremely low at only 17.00% through range

FT3: 4.4 pmol/l (Range 2.4 - 6)

Ft3 better at 55.56%

Helpful calculator for working out percentage through range

Many members on levothyroxine plus small dose T3 need BOTH Ft4 and Ft3 at least 50-60% through range

Your Ft4 is likely too low

Suggest you try increasing from 50mcg daily to 75mcg and 50mcg alternate days (or add 12.5mcg every day if you need unchanging Levothyroxine dose)

Just waiting for another T4 test result from 31/8 I thought T4 looked a bit low (Take in morning & didn't take morning of test 9am)

I didn't know if it was normal to be low when taking T3 as well.

Will speak to GP & Endocrinologist when get results of T4 from 31/8.

D3 tested 31/8 -137.9 Results stated >50 normal.

Was taking BetterYou D3000 + K2 when had test I was going to reduce from 3 sprays (3000) to 1 spray = 1000.

Do you think this is a good idea?
