hi guys

12 years ago i had hyperthyroid that went into hypo for a year after medication then went into remission

last 2 years have struggled with joint issues weight gain i have been diagnosed early 2021 with hypothyroid struggling to get bloods sorted have been given medication to help but feel like i am not recovering at all to the point i stopped taking it just before Christmas and not sure what to do next . also have a 6 cm fibroid and pre menopausal so have a lot going on . my biggest symptoms that effect my daily life are my joints honestly killing me some weeks am ok others like an old lady,

Request GP test vitamin D, folate, ferritin and B12 if not been tested

Add results and ranges if you have them

Frequently we have very low vitamin levels

How much levothyroxine are you currently taking

Bloods should be retested 6-8 weeks after each dose change or brand change in levothyroxine

hi i have had them all done my 12 was spot on but they also told me i was iron deficient but then contacted me 3 months later saying result was incorrect luckily i never took meds for that they gave me . i would look up my result and add them one here.

i was taking 50m of levothyroxine then 25m last 3 months and told i would be tested yearly .

my fibroid is just sat there hospitals discussed full womb removal . i have bloods on tuesday as i have pushed it .

my joint pain is ridiculous and effects me daily i rarely train atm because my body feels pretty broken

It's much, much better if you can post the actual numbers: results and ranges. Because words like 'spot on' are meaningless when talking about blood test results. They're just opinions not facts. :)

50mcg is only a starter dose levothyroxine

Do you mean levothyroxine dose was then increased to 75mcg

Bloods should be retested 6-8 weeks after each dose increase

Unless extremely petite…..Likely to need further increase in levothyroxine over coming months

Typically eventual dose levothyroxine is approx 1.6mcg per kilo of your weight per day

Roughly how much do you weigh in kilo

Which brand of levothyroxine are you currently taking

Many people find different brands are not interchangeable

Teva brand upsets many people. Teva is only brand that makes 75mcg tablets

Please add actual vitamin results

As greygoose says we frequently see these apparently “good vitamin results” are anything but good

Vitamin D at least around 80nmol and around 100nmol maybe better

Serum B12 at least over 500

Active B12 at least over 70

Folate and ferritin at least half way through range

hi i was on 25 microgram of levothyroxine to start then 50 micrograms then back down to 25 microgram brand is TEVA UK LTD all in space off 6 months my body weight is now 100 kg and rising i was 82 kg 18 months ago was a competitive powerlifter now i can hardly lift my body up and down the stairs due to joint pain

I would request different brand, my experience with TEVA brand was awful, I felt really unwell and now have DO NOT DISPENSE TEVA BRAND on my medical records.

Well you need to get FULL Thyroid and vitamin testing done 6-8 weeks after any dose change or brand change in levothyroxine

What were thyroid results after 50mcg …

dose should have been INCREASED by 25mcg …..not decreased

ALWAYS Test as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

How long have you been on just 25mcg

Are you taking any other medications or supplements

Levothyroxine doesn’t top up failing thyroid, it replaces it. That’s why almost everyone ends up on full replacement dose

Even if we frequently don’t start on full replacement dose, most people need to increase levothyroxine dose slowly upwards in 25mcg steps (retesting 6-8 weeks after each increase) until eventually on, or near full replacement dose

Traditionally we have tended to start patients on a low dose of levothyroxine and titrate it up over a period of months. RCT evidence suggests that for the majority of patients this is not necessary and may waste resources.

For patients aged >60y or with ischaemic heart disease, start levothyroxine at 25–50μg daily and titrate up every 3 to 6 weeks as tolerated.

For ALL other patients start at full replacement dose. For most this will equate to 1.6 μg/kg/day (approximately 100μg for a 60kg woman and 125μg for a 75kg man).

If you are starting treatment for subclinical hypothyroidism, this article advises starting at a dose close to the full treatment dose on the basis that it is difficult to assess symptom response unless a therapeutic dose has been trialled.

First thing is, do you have any actual blood test results? if not will need to get hold of copies.

You are legally entitled to printed copies of your blood test results and ranges.

The best way to get access to current and historic blood test results is to register for online access to your medical record and blood test results

UK GP practices are supposed to offer everyone online access for blood test results. Ring and ask if this is available and apply to do so if possible, if it is you may need "enhanced access" to see blood results.

In reality many GP surgeries do not have blood test results online yet

Alternatively ring receptionist and request printed copies of results. Allow couple of days and then go and pick up.

Important to see exactly what has been tested and equally important what hasn’t been tested yet

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

If TPO or TG thyroid antibodies are high this is usually due to Hashimoto’s (commonly known in UK as autoimmune thyroid disease). Ord’s is autoimmune without goitre.

About 90% of all primary hypothyroidism in Uk is due to Hashimoto’s. Low vitamin levels are particularly common with Hashimoto’s.

Gluten intolerance is often a hidden issue too. Request coeliac blood test BEFORE considering trial on strictly gluten free diet

reply hi i will request results again i have been tested for all intolerance my tummy swelled 2 years ago , i struggle with wheat milk but all my results were clear. have bloods tuesday but wont a private test

Well, you could get private labs done - your doctor will never do all the essentials, anyway, he sounds a bit clueless!

Hello Tammibunn :

Way back when told you were hyperthyroid and put on medication, was that an anti thyroid drug, like Carbimazole ?

Do you have the medical evidence from way back then or online access to view your medical history, and can see what you were dealing with - was there a diagnosis of Graves Disease and proof of positive and over range TSI or TR ab antibodies ?

hi 12 years ago they told me i was hyper took carbimazole i think then went low they then scanned my thyroid said it needed removing i refused .came back to uk test over a year was told it was full function .

2018 had nothing but issues tested for intolerances ect as my belly blew up i was sat at 80 kg all test came back for food intolerance . feb of 2001 after many blood test am pre menopausal scanned me have 6 cm fibroid i forced then to check thyroid 3 times have been told borderline asked them to medicate started on 25 m then 50 back down to 25 2 months ago . i will try and get hold of my medical records

You are legally entitled to copies of all your blood test results

Wheat/gluten and dairy/lactose intolerance all common with autoimmune thyroid disease (hashimoto’s)
