Oh gosh give me strength - Could only get an appt with student Dr this morning!

Hate the thing they do now - the receptionist asking u ton of questions before making a decision who u can see 🤷🏻‍♀️ First tried to give me an appt with nurse practitioner again 😑😑😑

You never know, you might be able to teach your student doctor a thing or two!If you can, tell him/her to look up ThyroidUK to learn more about thyroid problems.

It’s so hard - not sure how to go in without ruffling feathers ! Might try print out some of info give to me last 2 days to show

I’m not clear myself - just know I fed up feeling totally rubbish !

Keep it simple

YourTSH is too high for someone on levothyroxine and you would like/need a 25mcg dose increase in levothyroxine

Currently only on 50mcg (starter dose)

Increase to 75mcg

(But not a 75mcg tablet)

Request either 25mcg tablet or (if on Accord brand) an increase in number of 50mcg tablets per month and you will cut in half to get 25mcg dose. Accord don’t make 25mcg tablets

In the unlikely event they won’t give you an increase in dose, insist on retest of thyroid including Ft4, Ft3

Plus thyroid antibodies test

Student medical staff are not the same level of ‘student’ in other professions. They’d not be allowed to see a patient alone if they weren’t at that level of their learning, so I’d not worry about that so much. However they won’t be allowed to make any drastic changes without supervision/support so I’d probably say this appointment will just be monitoring/fact gathering.

They said the GP would be there too - not sure if meant actually there physically or to refer to ?

So, not so bad? I guess everyone’s got to learn but as this community knows, most of our interactions with GP’s or Endos look like they never learn, so in that way it doesn’t matter if they’re the student or the qualified or the postman! Lol

Nothing wrong with seeing student 's . (usually GP is in with them ,or just next door ) i had two students who get really exited that they' recognised a 'rare' skin condition .. ... they had to go get the GP to confirm diagnosis,, and prescribe ... but honestly they were much more interested in my skin than any GP had ever been .

I don't mind being asked what the problem is. Every request is put to a triage meeting, usually before 9am. Then it's decided if a telephone or face to face appointment would be best, and they decide who is the best one to deal with whatever the problem is. Some doctors have special interests so if I have a lung infection I'd rather be put with the GP who has a special interest in lungs rather than one who has a special interest in skin conditions, etc.

The very best one at our surgery wasn't a doctor, it was the Advanced Nurse Practioner, he was the only one who actually listened properly and had a full discussion with you and allowed you to discuss options. Sadly he has now left.

I had to see a student doctor earlier in the year, she was extremely thorough in her questioning and the supervising GP was brought in when she examined me. Went way beyond the normal 10 minute appointment. Couldn't fault her.

Came out with another sick note for depression n anxiety - 4 weeks 😭😭No increase of dose coz risk of stroke

Blood tests for antibodies and T4 agreed

The aim of levothyroxine is to increase dose upwards in 25mcg steps until TSH is ALWAYS under 2

When adequately treated, TSH will often be well below one.

Most important results are ALWAYS Ft3 followed by Ft4.

When adequately treated Ft4 is usually in top third of range and Ft3 at least 60% through range (regardless of how low TSH is)

Extremely important to have optimal vitamin levels too as this helps reduce symptoms and improve how levothyroxine works

Meanwhile test vitamin D

NHS easy postal kit vitamin D test £29 via

Optimal vitamin D is at least around 80nmol and around 100nmol may be better

If result is under 50nmol GP should prescribe 1600iu vitamin D daily for 6 months

If under 25nmol you need high LOADING dose of 300,000iu in total over 6-8 weeks

(5000iu daily for 8 weeks) and retest
