T4 - Levothyroxine is a storage hormone and your body needs to convert this into T3 which is the active hormone that the body runs on.
Your conversion of T4 into T3 can be compromised if your vitamins and minerals, especially ferritin, folate, and vitamins D and B12 are not maintained at optimal levels.
The NHS work to guidelines and ranges and some of these are too wide a range for hypothyroid people to sit somewhere, anywhere, and we need to be optimal with good, maybe higher ranges than the NHS sees necessary to supplement for, so yes, you mat need to build this essential core strength yourself.
The only way to know how well you covert the T4 into T3 is to have a blood test to take the measure of each, alongside your TSH, antibodies and ferritin, folate, B12 and vitamin D.
There are details of private companies who offer this service on the Thyroid uk website if your doctor declines and doesn't support you in your efforts to get better.
When with the results just start a new post on here, with the relevant ranges and you will be advised and helped to turn things around and find better health.
