Possible hypothyroidism?

Hi. I have recently been going through some testing for Addison’s disease due to low morning cortisol, which have so far come back normal. I have also been questioning hypothyroidism due to a big weight gain, headaches, dry skin, fatigue and a feeling of something stuck in my throat. I’ve now seen an NHS endocrinologist and a private one but both say my thyroid results are fine. I’ll post below if someone could help please? These tests were done in January and I have got progressively worse.

TSH 2.65 mu/L (0.34-5.6)

Free T4 9.3 pmol/L (7.7-15.1)

Thyroid peroxidase Ab <4 (<30)

Welcome to our forum.

It sounds like the beginnings of Hashimotos. TPOAb's are negative but you also need TGAb's tested to completely rule out. Doctors don't generally test for these. Also, thyroid antibodies are variable so better to test when you feel you are having a Hashi attack for a correct fluctuation result.

Cortisol issues are common in people with low thyroid hormone and your

FT4 is only 22% the way through range but TSH is fine. You also need FT3 tested for a complete picture of thyroid function and to rule out subclinical hypothyrodism.

Have you had VitB12, folate, ferritin and VitD tested ?

Hi. Thanks for replying. I have B12 deficiency, diagnosed 10 years ago and have Injections every 3 months. My vit D was 68nmol/L (>50 adequate level) folate 4.0 ug/L (3.8-25.0) Ferritin 46 ug/L (11-307) my last vit B12 was taken in 2017 and was 331 (>160 normal)

Those results are all less than optimal. Using UK units, vit D needs to be at least high 90s, folate in double figures, ferritin around 70 and B12 at least 500, pref at top of range (which is usually around 1000)

Thanks for replying. I’m already on B12 injections so I too thought that my B12 would have been higher. I don’t think the drs care as results come back in normal range so they’re not worried about them being on low side of normal.
