Test results come with a reference range, like this (123 - 456) this tells you whether you are in range or over/under range. We can't make a lot of sence from just a number.

Serum free T4 level 12.8 pmol/L 12 - 22pmol/L
Serum TSH level 2.76 miu/L 0.27 - 4.2miu/L
Ok, so tpo ab is above the reference range, ft4 is just above the bottom of the ref range, tsh is half way ish within range.
There are another set of antibodies but nhs do their best to ignor them because they dont understand the significance of them. There is also ft3 that is the most important measure but gp's aren't usually to order them.
As far as you are concered the tpo ab antibodies indicated that your immune system is damaging your thyroid.the damage is cumulative and will probably cause you problems at some point. These antibodies can be lowered by going totally glutten free. Because your ft4 is low in range so your ft3 will probably be low as well. Has doctor tested your nutrient levels, vit d vit b12 folate and ferretin? These are often low in people with thyroid problems and can lead to low levels of ft4 and ft3.

I have had all the signs of under active thyroid for many years and have always been told my results were ok. These set of results were only taking last week and its the first time the antibodies were tested.
I was told last year that I was low in B12 and to start taking a supplement, which I now do. I've just recently ordered Vitamin D.

That means that your antibodies are over range meaning that you have autoimmune thyroiditis (aka Hashimotos disease). Your free T4 is right at the bottom of the range (should be nearer 19) but your TSH isn't over range which you would expect with such a low free t4, so you might have a pituitary problem as well. Generally the NHS won't treat until your TSH is over 10 or free T4 under range, or you have high antibodies and symptoms and over range TSH. If you have a pituitary problem if could mean that your TSH can never rise so you might need a referral to an endo,
