Wher did the T4 go?

Just trying to understand my thyroid history and trying to make the right choice going forward. Could I just ask if these results from Jan 17 would indicate possible rT3 issues.

TSH 4.87. (0.27 - 4.20)

Free T4 21.37. (12 - 22)

FreeT3 4.35 (3.1 - 6.8)

It was at this point that I introduced T3 initially 6.25 and dropped my levothyroxine to 100 (from 125) increased to 12.5 and 100 and had been reasonably OK definitely better than in January.

My latest results

TSH 1.01. (0.27 - 4.20)

Free T4 13.7 (12 - 22)

FreeT3 4.2. (3.1 - 6.8)

What I can't understand is why after adding T3 my level has dropped slightly? I'm working on correcting any deficiencies (Vit d and ferritin) but is it worth considering RT3 - this is a blood draw and don't know if my latest results suggest it as a possibility at all.

I'm sorry for asking what might be the same question in different ways just trying to understand things 🙂

FT3 dropped because FT4 dropped from top of range to bottom of range. I don't think it was necessary to reduce Levothyroxine dose when you added T3 because FT4 wasn't over range and TSH was high.

High rT3 can be caused by high FT4 but since your FT4 has dropped it is unlikely rT3 is high. The only way to be sure is to have rT3 checked and you will require a phlebotomist to do a venous draw for that.

If you are still symptomatic there is scope to increase either Levothyroxine to 125mcg or T3 to 18.75mcg.

To me your initial results show a conversion issue, high FT4 and lower FT3 so you might have been better addressing that first as once you introduce T3 you can't check if that has been improved.
