What kind of adrenal issue

I have a suppressed TSH and take T4 which helped my symptoms improve. However, I still wake up at 3-4 am everyday and feel bad during the day, feel well at night.

but, I noticed recently whenever the adrenals do their normal job (like secretion of cortisol early morning) I do feel bad. What I noticed is when I drank water with high level of heavy metals from the tap while traveling, I got bad reactions and couldn't sleep because of my adrenals working to eliminate these heavy metals from my body.

Any Idea what is going with my adrenals?

Without an adrenal test its hard to say what part your adrenals are playing in how you feel. However, my understanding is that the elimination of toxins (whatever they may be) is through the liver, kidneys, skin, bowels and lungs. Blood sugar imbalances can cause you to wake up in the middle of the night.

Waking in the night can also be caused by poor breathing (not full sleep apnea, but just minor cessations).
