Good that he is saying that this is still under research and the breath tests are not accurate.
His protocol is initially treating with antibiotics and not dropping fibre, but going low fodmap, plus saccharomyces boulardii (which isn't bacteria but a beneficial funghi), then slowly reintroduce fodmaps makes sense plus slowly getting back to a position where you are much more fibre tolerant, which helps the microbiome in the long run. I 100% agree about not going down a road that destroys the microbiome - this is the protocol I was put on it and I became extremely ill from it (and am still suffering from the consequences of this). I also didn't have SIBO since my test was a false positive. I know this since none of the usual treatments worked.
Good that he is advocating for more research. I think one thing he didn't stress enough is attempting to find the root cause and treating that first. Otherwise you'll get over one bout which will automatically lead to another and another and an endless loop of treating it.
Antibiotics seem to be the nuclear option and Bill is rightly cautious that they should be used the right way. I doubt there's enough knowledge and confidence in NHS about using antibiotics to reign in SIBO, but the message on fibre is clear enough. Use it, but carefully (and unlike antibiotics you won't have to struggle to find the right guidance).
