Still persistent abdominal pain!

Hi all, I hope everyone had a lovely Christmas and hopefully a pain free one.I just wanted to see if anyone suffers with any intolerances? I'm at my wits end with this abdominal pain! It's really starting to concern me, I've had most tests done and nothing has been found up to now, I'm not convinced this pain is ibs. I've just got constant pain in my right lower side especially near my belly button and my stomach is constantly making loud noises, cramping and still having really loose stools. I'm wondering if anyone on here has celiac or something similar? I would be interested to know what there symptoms were and how long they went on for. I'm booked in to see a specialist to go through everything but I don't know when that will be as the doctors said it could take up to 2 month's. My doctors don't seem to concerned about it but it's sending my anxiety through the roof, I just can't seem to relax at all.

Hi, it could be a result/symptoms of bacterial overgrowth (dysbiosis) in your small bowel, your lower right abdomen. Have you ever tried, have you ever been given a course of a systemic antibiotic and if so do you remember if it helped (temporarily)? Best to discuss this with a gastroenterologist.

Bacterial overgrowth has crossed my mind a few times. I havnt been on any antibiotics for this issue. I will bring this to my gastro's attention.

Hi I’ve had lower to mid right side pain six years now every test, scan you name it I’ve had done all clear. Doc and me just don’t know. Some days good some bad …but it is frustrating ..

Hi bellacat21,

Yes it is very frustrating when u don't know whats happening with you're insides. I've had pretty much all tests done myself apart from the pill camera. I'm thinking that Will probably be the next step.

My pain was never really constant it would just flare up every now and again but these got more frequent. You could do with a stool test to check for inflammation. Not sure if you've had one of those yet. I had a pill camera which looks at your small bowel that found my issue

Hi viklou,

Yes I've had two stool test, the first results my calprotectin was high at 185 and then the second test my calprotectin was at 131. The pill camera sounds like the next thing I need to do. So far all my test have come back clear which is a good thing but very frustrating at the same time as I don't know whats going on with me.

You may be suffering from visceral hypersensitivity (functional abdominal pain) - there is info about it here:

It is where the brain interprets the normal activity of the bowel as pain - this is due to a wearing down of neurons in pain control centres of the brain which can be caused by PTSD, difficult childhoods, extreme stressful events etc. The first line treatment is nerve pain agents such as low dose amitriptyline. There is a theory that being on something like amitriptyline for 6-12 months can help the pain control centre neurons to regenerate. Note that amitriptyline can cause constipation, but this can be helpful in people who are diarrhea dominant. Unfortunately I couldn't tolerate these. Linaclotide (for IBS-C only) & Alflorex have helped me with this intestinal pain.

The reason why Alflorex may help is that the pain (and diarrhea) can be due to a gut bacterial imbalance which over sensitises the nerve endings in the gut. On top of disruption in the gut bran/axis (described above), this can give you a double whammy. You could try Alflorex in the first instance to avoid taking drugs.

You may also find assistance with anti-spasmodic medication such as mebeverine (Colofac) or enteric coated peppermint.

I will take a look in to this, thank you very much! I'm already taking mebeverine which works a little but in my opinion it's not fantastic. As for amitriptyline we have boxes of this stuff as my mrs suffers from endometriosis but I've never had any before.

Hi

I’m being tested for coeliac (the uk spelling) it’s a blood test but you have to be eating gluten at least 6 weeks prior to blood test my advice don’t cut out gluten if your being tested, my test is purely to rule it out, I don’t have a gallbladder so my symptoms could be to do with that as well as IBS, anxiety about our symptoms do not help in fact it makes it worse so try not to over think things, a over the counter medicine a GP told me to try for all IBS symptoms is COLPERMIN IBS relief it’s basically Peppermint oil I’ve tried it a couple of times with good results but we are not all the same are we, good luck with your appointment when it happens hope things start to settle down for you x

I will check this colpermin out and see if it helps, thank you. Yes I do have really bad anxiety so that can't be helping but I just get myself in a rutt with it at times. I have had the blood tests for celiac and everything seems normal, infact every test I've had has been normal, I just need some answers so I can deal with it.

Hello!

I’m afraid that IBS pain can genuinely be awful. That’s why IBS pain often leads to cancer investigations in the first instance. In your case, thankfully, they’ve found nothing that points to cancer, so it’s time to look at managing your symptoms. Yes, many of us on here have intolerances. For me, it’s lactose big time, but also gluten. Now that I cut those out, my pain is basically gone and my digestion feels better than it’s been in twenty years.

Have a look at the low fodmap diet. Ideally, you should have the help of a dietitian to guide you through it, but the NHS may or may not offer that for you. Alternatively, you could pay privately or manage it yourself. There is plenty of information on the internet. I found the low fodmap app from Monash University was extremely helpful. It cost ca. £6, and it’s a one-off cost which entitles you to a lifetime of updates, which is great value, really.

You could try a simple approach and just cut out lactose for two weeks, and see if it helps. If it doesn’t help, re-introduce it and try cutting out something else - maybe gluten. But the whole process is certainly easier with the advice of a dietitian.

Take measures to reduce the anxiety. Easier said than done, I know. Try and trust the process. They’ve checked for the scary stuff and found nothing. Try to trust in that. Take measures to improve your sleep. Go to bed at the same time every night. Do things that make you feel relaxed, whether that’s belting out an ABBA song or going for a walk - whatever works for you. I’m afraid anxiety only makes IBS worse, so anything you can do to reduce anxiety is worth doing. Low dose antidepressants can help relax you before sleep, and CBT may possibly help as well. Your gp can help with antidepressants and CBT is self-referral (google IAPT).

Anyway, quite a lot to think about there. Above all, know you’re not alone. There’s a whole community on here to help you and you’re welcome to ask questions anytime. Really hope things improve for you soon!

Hi singc,

Yes I have had pretty much every test going and nothing has been found, I completely agree with you that I need to look at that in a positive way, its just frustrating not knowing what the root cause is and in turn that causes my anxiety. I did try antidepressants but didn't stick with them as the made me feel weird but I may look in to them again or maybe get some different ones?

I have just started to cut gluten out, its my 3rd day there's seem to be a little improvement but not much but I will keep it up for a couple of weeks and see how I go. I will ask about seeing a dietitian.

Just a quick one to agree with the above poster! I had the most symptom relief from IBS from working with a dietician to do the Low Fodmap and taking a probiotic daily.

I realised I react to onions and garlic! Never would have guessed that they were a problem before doing the diet. My partner also has IBS and had a big reduction in pain when he cut out onion and garlic.

The diet is quite intense when doing the elimination, but it is really helpful to identify what might be triggering you off.

Thank you for you're input, I will definitely start to eliminate things and I will get a good look at this low fodmap diet going in to the new year. I've already started to cut out gluten, which has made a very small improvement but to early to tell yet.

I've had an ultrasound around 4 months ago to check for gallstones and nothing was found but obviously a scan is more accurate. I will put that forward to the specialist when I see him.

Hi maccaMine is very much the same but on the left side, pain there every day over the ladt 12months but since mid July a different type of pain when i move my upper body from the waist left or right, ive had tests including ct scan of the abdomen with contrast but when I see the gastro consultant on the 9th I'm going to ask if an mri scan would clear my daily constant fear and worry.

Sorry to hear you are suffering. Its crazy how symptoms can be persistent isn't it. That is also my main worry that it's been going on for months, can't imagine it going on for 12 months though, I truly do feel for you. I too hope to get some answers when I see my gastro and I hope you get the answers and the treatment you need. Keep me posted on what you're gastro says/recommends, thats if you don't mind.

I have ibs but a few years ago I discovered that I was also lactose intolerant, if I take to much lactose I can get severe stomach cramps that can last for 3-4 days.

It might be something to look into. After the first day without it I noticed a big difference in how I felt.

I am currently starting to cut things out of my diet, I have cut gluten out and also lactose. I will give it a good few weeks and then see how I feel. Not much improvement as of yet but I will keep going.

HiI cut out gluten from my diet for about 12 months, then slowly reintroduced it. Now I'm eating most things ok again except for onions. Onions are a massive no,no.

I had pain, and all the other lovely symptoms 😢 so doc tested me for all the regular stuff, all came back ok, except I wouldn't have the celiac test done because I was already gluten free at that time and was noticing a difference. I wasn't prepared to go through all that misery and pain for 6 weeks just to get a result, I knew gluten was the culprit.

Also, I had a tooth infection and needed antibiotics, and I noticed my stomach felt better afterwards,...maybe a coincidence, I don't know, but after that, I started to reintroduce gluten again, and now, like I said, I'm eating most things .

I also take a good probiotic every morning on an empty stomach., that's also helped enormously.

I am currently cutting gluten and lactose out of my diet, so I will see how I get on wih that. I'm not convinced it is that to be honest but I won't know unless I try. I've had all the tests which are negative, so I'm just confused what is going on really.

I have had every test fine all negative but was really suffering just before christmas my gp organised a poo test and on christmas day received a call to say had C Diff a very nasty bacteria infection in your bowels. Maybe get you GP to do this test as your symptoms are similar to mine.

I will put that forward to my gp when I see him next, I am thinking it's more along the lines of an infection rather than an intolerance. I have had many test which all have come back negative, so I'm still non the wiser which is very frustrating and worrying.

hi I’m coeliac - was diagnosed about 6 years ago but didn’t have very severe symptoms just abit of loose bowels no real pain - I had a blood test which is a good indicator of whether you might be (mine was very high) but they still like to do an endoscopy to see what your villi are doing ( if they’re flat you’re coeliac!) you have to keep eating gluten until the endoscopy else you will get a false reading possibly- have your blood showed anything or have you had a coeliac blood test yet because that’s your starting point thanks

I have had all blood tests done for any intolerances and also had an endoscopy around 4 weeks ago, that was all clear other than oesophagitis and a hiatus hernia. I have also had a colonoscopy around 7 months ago which was all clear also. I'm at my wits end with it and it's doing my anxiety any favours at all.

oh ok further down the line than I thought then - do you have IBS or take meds for this? The consultant thought I had autoimmune pancreatitis but personally I wonder whether my recent pain is ibs and have just started Colofac for it so hopeful this might help - I also take Lanszoprozole for excess stomach acid and found this made a big difference to my pain

I've had lots of test Cj75, stool samples, ultrasound, ct scan, colonoscopy, endoscopy, nasal endoscopy, numerous blood tests, fit tests, xrays. All tests have come back clear apart from my two recent stool samples as my calprotectin levels were quite high, 1st one was at 185 and the 2nd one was 131. Because of that I have to see a specialist but they said that could take up to 2 months to get see to, its already been a month now, I've still not heard anything from the hospital and still my symptoms persist.

Yes I'm on colofac, I find it did work in the beginning but now it dosnt have much effect, the same goes with buscopan. I find a hot water bottle helps more than those medications to be honest. I hope the colofac works for you. I am also on omeprazole for excess acid and hiatus hernia, it works a treat for acid reflux but as far as my bowel symptoms go, nothing touches it unfortunately.
