External RT to prostate when already mCRPC?
My dad failed on Zoladex and
Xtandi
. Had no RP, RT or docetaxel. Zytiga was stopped due to side effects. [u]Questions:[/u] Does anyone have experience with this RT approach? Is this a "debulking"? Can those 20 fractions remove the tumor or is it only a try to slow the growth?

To Chemo or Not to Chemo--that is the ?
Lost effectiveness so changed to
Xtandi
9/15/23. PSA: 9/25/23 = 26.6; 10/26/23 = 27.3, 11/23: 34.2; 1/5/24 = 49.9. New bone and CT scans 1/24/24 shows disease is progressing though changes are not enormous. I just don't know how to decide!

Summary of the following treatments: Bicalutamide this drug failed in July 2023 and the patient transitioned to
Xtandi
started in September 2023. PSA began to rise in January 2024 with a doubling time of 3.5 weeks, metastasis increased, pain increased, and urinary incontinence presented.

Need advice on clinical trial RO7656594
I have updated our profile, but in a nutshell we've gone through abiraterone,
Xtandi
, docetaxel, Pluvicto , cabazitaxel and carboplatin. My husband's most recent PSA on 4/12 was 79.52. His MO has put a hold on chemo our last 3 appointments due to pancytopenia.

PC no longer castrate resistant
I'm no longer taking
xtandi
, so it can be "washed out" of my system prior to new treatment, however I'm still getting quarterly lupron shots.

Next steps after PBRT
I’m on Orgovyx and
Xtandi
, with a plan to stay on them for 24 months total. Currently I’m undetectable as far as PSA goes. Jan 2024 will be 1 year since my last PSMA Pet. I’ve asked my doctor at Memorial Sloan Kettering for another scan this January, and he is pushing back stating it’s unnecessary.

Orchiectomy
I have been on Lupron
Xtandi
and Hi dose Vitamin C for the last 19 months my current PSA is <0.1 and has been for the last 6 months. I am 83 years old and am in reasonable good health, other than pc. I have read that having an orchiectomy would yield longer life. Also I could get off of Lupron.

A New complication!
On Decapeptyl 6 monthly injections, and over the years, Xgeva,
Xtandi
and for the past couple of years, Erleada. Unfortunately, due to stomach surgery 2 years ago, where his vagus nerve was cut, he has now developed gastroporesis.

calcium supplement
I would like to know, if it’s beneficial to take calcium 1200 mg with 1000u D3 for someone who is on trelstar,
xtandi
and completed radiation. Dexa scan shows osteoarthritis. Stage 4 also with pacemaker. I am asking specifically about calcium, I am aware of D3 studies .

Help to do BAT treatment for my dad. Located in Norway.
September he was put on
Xtandi
which unfortunately gave him horrible side effects and we had to stop. His cancer has now starting to spread to his vertebrae, pelvis, femur and 3 lymphnodes in his chest cavity. This was treated with radiation therapy 8 weeks ago.

dad has advanced prostate cancer
He’s beeen on Lupron and
xtandi
and some other meds that gave him confusion. At this point he’s 83, it’s in a few ribs, hip (he had hip replacement a few years ago), clavicle, base of skull, spinal areas, spots on lung and liver. I think i covered it all.

Zytiga after failing nubeqa ?
I had some good results years ago when I was still nmCRPC with Firmagon/

, but had to stop due to toxicities.

Enzalutamide (Xtandi): Start at half standard dose to extend efficacy time span?

160 mg/di in April which pushed PSA back down to <0.1 but ALT liver enzyme up to 560 (ULN 59). This forced stopping

for 4 weeks until ALT was down to 69. Restarted

4 weeks ago on 80 mg/di. Today ALT 38 and PSA still <01.

Abiraterone and Breathlessness
My Oncologist tells me the NHS will no longer approve changing to

, and he clearly thinks that Abiraterone is the villain of the piece. Meanwhile, my PSA continues to be <0.1.

Adding second line hormonal therapy?
I suppose adding newer drugs (such as Zytiga,
Xtandi
, ...) will be better as they are newer and more efficient. Also, how concerned should we be about PSA rise?

My experience with Docetaxel
Before I started I had already been on Lupron for five years, I had radiation to multiple bone Mets, took zytiga/
xtandi
, had Provenge and did a clinical trial. October 26 2023 I started chemo as my cancer was progressing. My PSA at the start was 17.

Lymparza not working
He did very well on
Xtandi
but was ended this past Aug when Penn saw progression but the trial folks did not so no cross over allowed. After several more scans he still showed more progression but trial folks still did not so he dropped out and went on Lymparza end of Aug.
