Hi all - First and foremost, Happy Holidays. I had my prostate removed 8/22, with subsequent Proton Beam treatment ending 4/23 (after PSA did not drop to undetectable) brought upon after a mild uptake (during a Petscan) in an obturator lymph node. I’m on Orgovyx and Xtandi, with a plan to stay on them for 24 months total. Currently I’m undetectable as far as PSA goes. Jan 2024 will be 1 year since my last PSMA Pet. I’ve asked my doctor at Memorial Sloan Kettering for another scan this January, and he is pushing back stating it’s unnecessary. Curious to know from you all if you think I should continue to press him or is it too early, and if so, when would be ideal?

He is right -- it won't show anything useful. You will be on Lupron+Xtandi for 2-3 years. After that, if your PSA rises, you may have another decision to make. As of now, there are no decisions to make.
Thanks Allen - is there any truth that 2nd gen’s such as Xtandi may show a low PSA, but in fact there is a potential that further growth may occur? I’ve listened to Dr Kwon from Mayo during his latest release where he mandates his patients get a scan, even if PSA is low. For some context, my PSA was only single digits (4.1) when confirmed with Gleason 8 with Cribiform. I guess my concern is that I don’t want to be caught in that small percentage of people that assume all is ok based off of PSA only, given my numbers were never really “high” to begin with. Decipher was at .86 . Trying to find a happy medium with using scan technology along side PSA.
